The study found that the geriatric nutritional risk index (GNRI) and the Charlson comorbidity index (CCI) were predictors of 3-year survival. The established prediction model using GNRI and CCI can assist in treatment decision-making for elderly patients with EGC aged >= 85 years.

Predictors of early and late mortality after the treatment for early gastric cancers

Summary: This study aimed to clarify the predictors of early and late mortality in patients with early gastric cancers (EGCs). Factors such as age, Eastern Cooperative Oncology Group Performance Status, Charlson comorbidity index, American Society of Anesthesiologists Physical Status, and C-reactive protein/albumin ratio were identified as important predictors for both early and late mortality. Additionally, male gender, intermediate/high-risk of LNM in eCura system, and sarcopenia evaluated by the psoas muscle mass index were predictors for early mortality. The results suggest that a combined assessment of multiple predictors may help predict mortality outcomes in EGC patients.

Long-term outcomes after endoscopic resection for late-elderly patients with early gastric cancer

Recent approach for preventing complications in upper gastrointestinal endoscopic submucosal dissection

Summary: This article introduces the methods and techniques for preventing complications during upper gastrointestinal endoscopic submucosal dissection (ESD). In esophageal and gastric ESD, intraoperative perforation is a major complication that can be prevented by avoiding blind dissection and using traction-assisted ESD. Delayed bleeding is the most common complication in gastric ESD, and a prediction model (BEST-J score) has been established to assess the risk of delayed bleeding. Vonoprazan >= 20 mg/day is currently the only reliable method for reducing delayed bleeding in gastric ESD. Duodenal ESD has a higher frequency of complications, such as perforation and delayed bleeding, but advancements in devices and techniques have decreased the occurrence. Several ESD techniques, including the use of the distal tips of the Clutch Cutter, can help prevent intraoperative perforation. Standardization of duodenal ESD and further studies are still needed.
