Heart Failure With Preserved Ejection Fraction in Hypertension Patients: A MyocardialMRStrain Study

Summary: This retrospective study aimed to use feature-tracking (FT) to detect and evaluate heart failure with preserved ejection fraction (HFpEF) in patients with hypertension (HTN), and determine optimal strain cutoffs for the diagnosis of HFpEF-HTN. The study found that HFpEF-HTN patients exhibited significantly impaired left ventricular strains compared to HTN patients and healthy controls, with global peak systolic longitudinal strain (GLS) showing the highest diagnostic value for differentiation. FT-derived strain parameters have potential value for the diagnosis and risk stratification of HFpEF-HTN patients.

Summary: Myocardial fibrosis, characterized by the expansion of cardiac interstitium through deposition of extracellular matrix proteins, is a common pathophysiologic feature in various myocardial conditions. Activated fibroblasts and myofibroblasts play a central role in cardiac fibrosis, producing matrix proteins and triggering fibrogenic signalling cascades in response to stress. Immune cells, vascular cells, and cardiomyocytes can also contribute to fibrosis, while fibrotic changes may disrupt cardiac function and play a role in arrhythmogenesis.
