Meta-Analysis of Bioprosthetic Valve Thrombosis After Transcatheter Aortic Valve Implantation

Summary: Bioprosthetic valve thrombosis is common after TAVI, but does not increase the risk of death. Oral anticoagulation can reduce the prevalence of bioprosthetic valve thrombosis, while clinical valve thrombosis is significantly associated with an increased risk of stroke. Future studies should focus on prevention and treatment of bioprosthetic valve thrombosis.

Summary: This meta-analysis evaluated the clinical relevance of subclinical leaflet thrombosis (SLT) post transcatheter aortic valve replacement. The use of intra-annular valves was associated with a higher risk of SLT, while oral anticoagulation showed a significant risk reduction. Further studies are needed to assess the long-term effects of screening tests and antithrombotic therapy on valve functionality and clinical prognosis.

Summary: Transcatheter aortic valve implantation (TAVI) is an effective treatment for older patients with symptomatic severe aortic stenosis, and its indication has expanded to younger patients at lower risk. Despite a decrease in thromboembolic and bleeding complications after TAVI, these adverse events are still common. Recommendations for antithrombotic therapy in patients undergoing TAVI are mainly based on expert opinion, while recent studies and trials provide updated insights on antithrombotic treatment during and after the procedure.
