Underlying genetic etiologies of congenital diaphragmatic hernia

Summary: This article reviews the genetic abnormalities, copy number variants, and single genes associated with congenital diaphragmatic hernia (CDH). It suggests using array-based copy number variant analysis for identifying chromosomal abnormalities and copy number variants, and whole exome sequencing for identifying causative sequence variants. Whole genome sequencing with copy number variant analysis has the potential to become the most efficient means of identifying underlying diagnoses, but is not yet routinely available.

Early, Postnatal Pulmonary Hypertension Severity Predicts Inpatient Outcomes in Congenital Diaphragmatic Hernia

Summary: The study found that early CDH-PH is associated with increased risk for inpatient mortality or oxygen support at 30 days. Infants with moderate CDH-PH and severe CDH-PH had a higher incidence of the primary outcome, while severe CDH-PH was also associated with a higher likelihood of requiring ECLS support.

Summary: This study evaluated potential risk factors for mortality of infants with congenital diaphragmatic hernia, finding that right-sided hernia, treatment with inhaled nitric oxide over 15 days, and the use of ECMO over 10 days were significant risk factors for mortality.

Pulmonary vasodilators have become available for use in neonates thanks to improved understanding of pulmonary hypertension pathobiology, but a comprehensive algorithm to guide treatment of refractory PPHN is still lacking.
