Clinical Efficacy and Nephrotoxicity of the Loading Dose Colistin for the Treatment of Carbapenem-Resistant Acinetobacter baumannii in Critically Ill Patients

Summary: The aim of this study was to assess the 30-day survival and nephrotoxicity in critically ill patients with carbapenem-resistant Acinetobacter baumannii (CRAB) infection who received non-loading dose (LD) versus LD of colistin methanesulfonate (CMS). The study found that the LD CMS group had higher survival rates, clinical response, and microbiological response compared to the non-LD CMS group. However, LD CMS was associated with an increase in nephrotoxicity, requiring close monitoring of renal function.

The findings suggest that implementing proper management strategies and reducing the unreasonable use of antibacterial drugs may help in reducing the spread of carbapenem-resistant Gram-negative bacteria.

The dominant carbapenemase gene was KPC-2, and the main CRKP sequence type shifted from ST11 to ST15, resulting in increased sensitivity to amikacin.
