Current and emerging disease-modulatory therapies and treatment targets for multiple sclerosis

F. Piehl

Summary: Treatment of multiple sclerosis is evolving with the approval of novel and increasingly effective disease-modulatory therapies. In clinical development, inhibitors of Bruton's tyrosine kinase represent a new emerging drug class in MS. Accumulating evidence suggests the importance of early high effective therapy while maintaining acceptable tolerability.

Oligoclonal IgM bands in the cerebrospinal fluid of patients with relapsing MS to inform long-term MS disability

Rocco Capuano et al.

Summary: This study investigated the role of cerebrospinal fluid (CSF) markers in predicting long-term multiple sclerosis outcomes, with the presence of oligoclonal IgM bands (OCMB) associated with unfavorable long-term outcomes. These findings suggest that OCMB should be determined in relapsing MS patients to inform long-term prognosis.

Cerebrospinal Fluid IgM and Oligoclonal IgG Bands in Multiple Sclerosis: A Meta-Analysis of Prevalence and Prognosis

Mattia Fonderico et al.

Summary: The presence of intrathecal IgM synthesis (ITMS) is associated with an aggressive clinical course of multiple sclerosis (MS), being higher in relapsing-remitting (RR) patients. The risk of second relapse is higher in OCGBs positive patients, but much higher in ITMS positive patients. This study suggests that the risk of a second relapse may be related to the presence of intrathecal IgM to some extent.

Intrathecal Immunoglobulin M Synthesis is an Independent Biomarker for Higher Disease Activity and Severity in Multiple Sclerosis

Johanna Oechtering et al.

Summary: In relapsing multiple sclerosis, intrathecal synthesis of IgM is significantly associated with disease activity and severity, leading to shorter time to relapse, increased severity, higher sNfL concentrations, more lesions, and other characteristics.

Correlation between contrast enhanced plaques and plaque diffusion restriction and their signal intensities in FLAIR images in patients who admitted with acute symptoms of multiple sclerosis

Amin Abolhasani Foroughi et al.

Summary: This study evaluated the correlation between DWI signal restriction, contrast-enhancement, and demyelinated plaque's signals in MS patients. The sensitivity and specificity of DWI restriction in detecting active plaques were calculated, with an accuracy of 92.91%. LWR was found to have higher sensitivity and specificity compared to LNR, making it a better index in detecting active plaques.
