The Impact of Depression and Exercise Self-Efficacy on Benefits of Pulmonary Rehabilitation in Veterans with COPD

Patricia M. Bamonti et al.

Summary: This study examines the impact of depression symptoms and exercise self-efficacy on pulmonary rehabilitation outcomes. The results suggest that reducing depression symptoms is associated with greater improvement in health-related quality of life and certain aspects of exercise capacity. Baseline exercise self-efficacy also moderates the association between depression symptoms and changes in fatigue levels. Addressing mood and confidence to exercise is important for successful pulmonary rehabilitation programs.

Mental Health Needs of Aging Veterans: Recent Evidence and Clinical Recommendations

Michele J. Karel et al.

Summary: The mental health needs of older Veterans are complex and deserve attention. While most older Veterans are resilient, a significant minority experience mental health concerns, including a higher risk of suicide. Older Veterans are less likely to utilize mental health services compared to younger Veterans, but can benefit from treatment once engaged.

Rapid Increase in Telemental Health Within the Department of Veterans Affairs During the COVID-19 Pandemic

Samantha L. Connolly et al.

Summary: During the COVID-19 pandemic, there was a rapid growth in TMH-V appointments within the Department of Veterans Affairs, with 77.5% of patients being first-time users. The growth rate of TMH-V appointments was higher than telephone appointments, while in-person appointments decreased significantly. The existing telehealth infrastructure in VA played a key role in facilitating the rapid expansion of TMH-V appointments during the pandemic.

Home-Based Cardiac Rehabilitation EXPERIENCE FROM THE VETERANS AFFAIRS

Kariann R. Drwal et al.

Summary: The study evaluated the effectiveness and safety of the Veterans Affairs national home-based cardiac rehabilitation (HBCR) program. Results showed significant improvements in exercise capacity, depression, cardiac self-efficacy, body mass index, and eating habits among participants, with high satisfaction levels. The VA HBCR program demonstrated strong evidence of effectiveness and safety for a wide range of patients, indicating its potential as an adjunct to site-based programs. Further research is needed to assess long-term effects, cost-effectiveness, and sustainability of the model.

Molecular markers of aging, exercise capacity, & physical activity in COPD

Summary: The study suggests that epigenetic measures of biological age are independent predictors of exercise capacity and physical activity in COPD patients, but not associated with acute exacerbations. Adjusted models show that changes in biological age are gradually associated with exercise capacity and physical activity.

Pharmacological treatment of stable chronic obstructive pulmonary disease

Dave Singh

Summary: Pharmacological treatment for COPD has moved towards a personalized approach, integrating clinical and biomarker information to optimize treatment decisions for individuals. Blood eosinophil counts have been adopted as a biomarker to predict responses to ICS in COPD patients. The development of novel inhaled combination treatments has provided valuable evidence to guide the use of different classes of combination treatments.

Jenny Lam et al.

Summary: Analysis of the Veterans Health Administration (VHA) database revealed that roflumilast was associated with higher hazard ratios for death, COPD-related hospitalization, and all-cause hospitalization in COPD patients, calling for further prospective clinical trials to directly compare the relative efficacy of these therapies.

Summary: This study evaluated the effectiveness of a web-based self-management intervention focused on physical activity promotion in COPD patients. While the intervention did not improve exercise capacity, it did lead to increased physical activity. Further intervention modifications may be needed to optimize COPD self-management capabilities.

Long-term Exercise After Pulmonary Rehabilitation (LEAP): a pilot randomised controlled trial of Tai Chi in COPD

Marilyn L. Moy et al.

Summary: This pilot study suggests that Tai Chi may be a feasible option to help individuals with COPD maintain the benefits gained after completing conventional pulmonary rehabilitation. Participants in the Tai Chi group showed improvements in exercise capacity, health-related quality of life, fatigue, and control compared to those in the usual care group.

Kristen M. Kraemer et al.

Summary: In this pilot feasibility randomized controlled trial, tai chi showed greater improvements in depressive symptoms, 6-minute walk test distance, social support, and chair stand compared to the MBB intervention. While mindful breathing may not have significant impact, improvements in social support were maintained at the 24-week follow-up. Further research is needed to understand the effects of mindful breathing for COPD and strategies to maintain benefits from tai chi over time.
