A Randomized Trial of Point-of-Care Early Infant Human Immunodeficiency Virus (HIV) Diagnosis in Zambia

Summary: A study conducted in Zambia showed that point-of-care (POC) early infant diagnosis (EID) can eliminate diagnostic delays and accelerate the initiation of antiretroviral therapy (ART). However, despite a high rate of ART initiation, the results did not show significant improvement in 12-month outcomes. Therefore, in settings where centralized EID is well functioning, POC EID is unlikely to improve pediatric HIV outcomes.

Point-of-care p24 antigen detection for early infant diagnosis of HIV infection: cross-sectional and longitudinal studies in Zambia

Summary: This study evaluated the validity of a novel point-of-care p24 antigen detection test (LYNX) in rural and urban settings in southern Zambia for early infant HIV diagnosis. With high specificity and increasing sensitivity with age, the LYNX test could be valuable for early diagnosis in infants >= 4 weeks old, particularly in rural areas. Development of affordable, easy-to-use point-of-care tests with moderate sensitivity and high specificity is needed to improve access to testing, especially in areas where nucleic acid-based point-of-care assays are not feasible.
