Deep brain stimulation targets in epilepsy: Systematic review and meta-analysis of anterior and centromedian thalamic nuclei and hippocampus

Further research is needed to explore the efficacy of DBS for other targets such as the centromedian thalamic nucleus and hippocampus. Factors such as electrode location, stimulation parameters, type of epilepsy, and duration of stimulation are associated with clinical outcomes. Advances in anatomical targeting, functional neuroimaging, and sensing technology could potentially improve outcomes of DBS for epilepsy.

The role of thalamic nuclei in genetic generalized epilepsies

Summary: This review summarizes data on the participation of different thalamic nuclei in genetic generalized epilepsies (GGE). The traditional 'three compartment model' on seizure generation is considered too simplistic as the thalamus cannot be seen as a unity. Different thalamic nuclei integrated in different thalamocortical and subcortical loops play different roles in seizure generation, generalization, and maintenance. The networks involved may vary between different classes of GGE patients and can even be antagonistic between seizure types.
