Severe acute respiratory coronavirus virus 2 (SARS-CoV-2) seroconversion and occupational exposure of employees at a Swiss university hospital: A large longitudinal cohort study

Romain Martischang et al.

Summary: The seroconversion proportion of hospital employees increased during the study period, especially in employees working in COVID-19 geriatrics and non-COVID-19 geriatrics wards. Nursing assistants had a significantly higher risk of seroconversion compared to office workers. Significant risk factors for seroconversion included the use of public transportation, community exposure, nosocomial COVID outbreak, and working in geriatrics wards.

The role of viral genomics in understanding COVID-19 outbreaks in long-term care facilities

Dinesh Aggarwal et al.

Summary: This study reviewed all genomic epidemiology studies on COVID-19 in long-term care facilities (LTCFs) and found that staff and residents usually had identical or very similar SARS-CoV-2 genomes. Outbreaks typically involved one main cluster, and the same lineages persisted despite infection control measures. It was also observed that the sequencing technique was reliable over time, and when combined with local epidemiology, genomics could help identify probable transmission sources. Multiple studies detected transmission between LTCFs, and the mortality rate among residents was high regardless of the lineage.

The contribution of hospital-acquired infections to the COVID-19 epidemic in England in the first half of 2020

Gwenan M. Knight et al.

Summary: This study found that SARS-CoV-2 transmission in hospitals contributed to approximately a fifth of identified cases of hospitalized COVID-19 during the first wave in England, but accounted for less than 1% of all infections in the country. Using time to symptom onset from admission as a detection method likely missed more than 60% of hospital-acquired infections.

Whole-genome Sequencing to Track Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) Transmission in Nosocomial Outbreaks

Mary Lucey et al.

Summary: This study utilized whole virus genome sequencing to analyze hospital-acquired COVID-19 cases, revealing that transmission primarily occurred among patients dependent on nursing care through close contact or droplet transmission. The majority of cases were transmitted from healthcare workers to patients, highlighting the importance of interventions like comprehensive screening and universal use of surgical masks to prevent viral spread in healthcare settings. Collaboration between guidance bodies and IPC experts is crucial for developing effective control measures in emergency pandemic situations.

Nosocomial transmission and outbreaks of coronavirus disease 2019: the need to protect both patients and healthcare workers

Mohamed Abbas et al.

Summary: The COVID-19 pandemic has significantly impacted hospitals and healthcare providers worldwide, increasing the risk of nosocomial transmission and outbreaks to non-COVID patients or residents, who are at the highest risk of mortality. Reports on nosocomial outbreaks of SARS-CoV-2 have shown variable attack rates of up to 60%, with high mortality. There is limited evidence on transmission dynamics and the role of healthcare workers in initiating or amplifying nosocomial outbreaks, but there has been a shift towards expanded testing criteria for patients and healthcare workers, including asymptomatic individuals, which appears crucial in identifying and controlling outbreaks.

Viral Sequencing to Investigate Sources of SARS-CoV-2 Infection in US Healthcare Personnel

Katarina M. Braun et al.

Summary: The study found that the majority of healthcare personnel (HCP) infections were not linked to healthcare-associated transmission but rather were genetically similar to viruses circulating in the community. While not ruling out the possibility of healthcare-associated transmission, it emphasizes the importance of mask-wearing, social distancing, robust testing programs, and rapid vaccine distribution.

Summary: In England, hospital-onset probable and definite cases accounted for 5.3% of all laboratory-confirmed COVID-19 cases, with community-onset community-acquired cases being the highest proportion. Hospital-associated SARS-CoV-2 infections were identified in single and multi-site hospital-based studies, with up to 1 in 6 infections among hospitalised patients being attributed to nosocomial transmission.

Summary: Nosocomial transmission of SARS-CoV-2 within hospitals is a major concern, with approximately 20% of infections in inpatients and 73% of infections in healthcare workers being attributed to nosocomial transmission. Strategies such as isolating suspected COVID-19 patients in single rooms or bays can significantly reduce hospital-acquired infections in patients.

Summary: The study developed a novel statistical method and sequence reporting tool (SRT) that integrates epidemiological and sequence data to rapidly assess the probability of HCAI among HOCI cases, providing useful feedback for infection prevention and control teams. Compared to traditional phylogenetic methods, this approach may be more effective in understanding potential transmission clusters of SARS-CoV-2.

Joseph Ottolenghi et al.

Summary: The study explored healthcare workers' attitudes towards patients infected with SARS-CoV-2 in two highly affected hospitals in New York during the early stage of the pandemic, revealing high levels of stress and concern among the workers. Despite this, the majority of them were willing to treat patients with SARS-CoV-2 infection. Registered nurses and healthcare workers living with children were more likely to believe it was ethical to refuse care for SARS-CoV-2-infected patients.
