Androgen deprivation therapy and risk of cognitive dysfunction in men with prostate cancer: is there a possible link?

Summary: The use of androgen deprivation therapy (ADT) to treat prostate cancer has increased over the recent decades, leading to improved patient survival. However, the long-term implications of ADT, including cognitive dysfunction, are becoming more apparent. Previous studies have suggested a link between ADT and cognitive decline, but the results have been inconsistent due to limitations in study design and data analysis. This review article summarizes key findings and discusses the results of several studies investigating the association between ADT and cognitive dysfunction.

Summary: Chemotherapy-related cognitive complaints are common in breast cancer patients, but the neural bases for these complaints are not well-understood. A pilot study showed that chemotherapy-treated breast cancer patients with cognitive complaints exhibited increased functional connectivity between specific brain regions, which was negatively correlated with executive function and attention. These findings suggest that cerebellar-cortical functional connectivity changes may contribute to chemotherapy-related cognitive complaints and indicate potential minor cognitive function impairment in breast cancer patients.

Summary: Alterations in spontaneous brain activity and functional connectivity were observed in post-stroke cognitive impairment patients, particularly in the default-mode network and the salience network. The decreased fALFF and FC values in patients with hemorrhagic and ischemic stroke may serve as the pathological mechanism of cognitive impairment. Additionally, fALFF values and functional connectivity maps can be used to specify a target map for repetitive transcranial magnetic stimulation on the cortical surface.
