Intraoperative Neuromonitoring of Blink Reflex During Posterior Fossa Surgeries and Its Correlation With Clinical Outcome

Elif Ilgaz Aydinlar et al.

Summary: This study aimed to test the utility of intraoperative blink reflex (BR) during cerebellopontine angle and skull base surgeries, assess its prognostic value for facial nerve functions, and compare it with facial corticobulbar motor evoked potentials (CoMEPs). The study found that BR had a slightly better sensitivity than facial CoMEPs in predicting impairment of facial nerve functions for both postoperative and third-month time points. The blink reflex showed better accuracy for predicting postoperative nerve functions, whereas CoMEPs correlated better in predicting third-month outcome.

Continuous Laryngeal Adductor Reflex Versus Intermittent Nerve Monitoring in Neck Endocrine Surgery

Catherine F. Sinclair et al.

Summary: Continuous RLN monitoring using LAR significantly reduced rates of postoperative transient VF paralysis and paresis compared to intermittent IONM. Surgeons should consider the benefits of continuous intraoperative monitoring for preventing nerve injury and improving patient outcomes.

Trends in the Management of Non-Vestibular Skull Base and Intracranial Schwannomas

Carlos Suarez et al.

Summary: The aim of this review is to analyze the latest trends in the management of non-vestibular skull base and intracranial schwannomas in order to optimize tumor control and quality of life. Management decisions should be individualized depending on tumor size, location and associated functional deficits. Generally, large sized schwannomas exerting significant mass effect with increased intracranial pressure are treated surgically.

Neurophysiological monitoring of the laryngeal adductor reflex during cerebellar-pontine angle and brainstem surgery

Maria J. Tellez et al.

Summary: This study evaluated the impact of posterior fossa and brainstem surgery on laryngeal function by monitoring LAR and vocal-CoMEPs. It found that permanent LAR-SC or loss correlated with postoperative laryngeal dysfunction, while timely surgical adjustments could prevent irreversible damage.
