The enigma of persistent symptoms in hypothyroid patients treated with levothyroxine: A narrative review

Petros Perros et al.

Summary: A significant minority of patients with hypothyroidism continue to experience symptoms despite normal thyroid biochemistry, and this can be attributed to imperfections in treatment methods, coexistence with somatic symptom and related disorders, autoimmune neuroinflammation, and comorbidities and psychosocial factors. Currently, there is a bias towards pursuing answers related to combination treatment of levothyroxine and liothyronine, while other explanations have been neglected.

Real-life practice of thyroid hormone use in hypothyroid and euthyroid patients: A detailed view from the THESIS questionnaire survey in France

Camille Buffet et al.

Summary: This study aimed to analyze the practices of French physicians in thyroid hormone therapy, specifically focusing on the use of various LT4 formulations. The majority of physicians in France prefer levothyroxine (LT4) as the treatment of choice for hypothyroidism. However, some physicians consider using liothyronine (LT3) or a combination of LT4 and LT3 when hypothyroidism symptoms persist despite normal TSH levels. There is also a significant proportion of physicians who would consider treating euthyroid patients, contrary to current knowledge. They also found that LT4 tablets were the most preferred formulation.

Summary: The majority of specialists in Greece prefer levothyroxine (LT4) as the first-line treatment for hypothyroidism. Soft-gel capsules are favored for patients with food intolerance, generic LT4 use, and unexplained poor biochemical control, or inability to take LT4 fasted and separate from food/drink. The use of combined LT4 + LT3 therapy is rare, with most specialists only considering it for specific cases such as patients recovering from protracted hypothyroidism or those with persistent symptoms despite normal serum TSH levels.

Primary hypothyroidism and quality of life

Laszlo Hegedus et al.

Summary: The treatment of hypothyroidism has not seen major innovations since the 1970s when thyroid extract was replaced by levothyroxine. The definition of subclinical hypothyroidism is debated and indiscriminate screening has led to overdiagnosis and treatment initiation at lower TSH levels. While levothyroxine substitution can improve quality of life, it may not always normalize it, especially for individuals with mild hypothyroidism.

Irfan Vardarli et al.

Summary: German thyroid specialists prefer LT4 as the treatment of choice for hypothyroidism and consider combination therapy with LT4+LT3 for patients with persistent symptoms. They prefer LT4 tablets over other formulations and do not support the widespread use of thyroid hormones for non-hypothyroid conditions.

Use of thyroid hormones in hypothyroid and euthyroid patients: a 2020 THESIS questionnaire survey of members of the Czech Society of Endocrinology

Jan Jiskra et al.

Summary: In the Czech Republic, most endocrinologists use levothyroxine (LT4) as the primary treatment for hypothyroidism. However, there is also a significant percentage of practitioners who use liothyronine (LT3) in clinical practice and believe that thyroid hormones may be indicated in biochemically euthyroid patients. Additionally, there is a liberal approach to prescribing thyroid hormones to euthyroid patients and using LT4/LT3 combination treatment for hypothyroid patients with persisting symptoms, despite variance with guideline recommendations.

Summary: In Hungary, LT4 is the preferred treatment for hypothyroidism. Combination therapy with LT4 + LT3 is considered for patients with persistent symptoms. In the presence of comorbidities and interfering medications affecting bioavailability, many Hungarian endocrinologists prefer the new LT4 formulation.

Summary: The majority of Polish physicians prescribe thyroid hormones for euthyroid patients for certain indications, and some may consider using LT4+LT3 combination therapy when symptoms persist. Certain areas of clinical practice, such as the use of thyroid hormones in euthyroid subjects, do not align with current evidence.

Summary: The study found that the preferred treatment for hypothyroidism in Sweden is L-T4 tablets, with around 50% of endocrinologists also prescribing L-T3 or a combination of L-T4+L-T3. In euthyroid patients, about half of the respondents believed that thyroid hormone treatment was not necessary, while nearly half would consider prescribing L-T4 for euthyroid infertile women with high TPO antibody levels.
