Summary: A significant minority of patients with hypothyroidism continue to experience symptoms despite normal thyroid biochemistry, and this can be attributed to imperfections in treatment methods, coexistence with somatic symptom and related disorders, autoimmune neuroinflammation, and comorbidities and psychosocial factors. Currently, there is a bias towards pursuing answers related to combination treatment of levothyroxine and liothyronine, while other explanations have been neglected.

Summary: This study aimed to analyze the practices of French physicians in thyroid hormone therapy, specifically focusing on the use of various LT4 formulations. The majority of physicians in France prefer levothyroxine (LT4) as the treatment of choice for hypothyroidism. However, some physicians consider using liothyronine (LT3) or a combination of LT4 and LT3 when hypothyroidism symptoms persist despite normal TSH levels. There is also a significant proportion of physicians who would consider treating euthyroid patients, contrary to current knowledge. They also found that LT4 tablets were the most preferred formulation.

The use of combined LT4 + LT3 therapy is rare, with most specialists only considering it for specific cases such as patients recovering from protracted hypothyroidism or those with persistent symptoms despite normal serum TSH levels.

Summary: The treatment of hypothyroidism has not seen major innovations since the 1970s when thyroid extract was replaced by levothyroxine. The definition of subclinical hypothyroidism is debated and indiscriminate screening has led to overdiagnosis and treatment initiation at lower TSH levels. While levothyroxine substitution can improve quality of life, it may not always normalize it, especially for individuals with mild hypothyroidism.

Summary: In the Czech Republic, most endocrinologists use levothyroxine (LT4) as the primary treatment for hypothyroidism. However, there is also a significant percentage of practitioners who use liothyronine (LT3) in clinical practice and believe that thyroid hormones may be indicated in biochemically euthyroid patients. Additionally, there is a liberal approach to prescribing thyroid hormones to euthyroid patients and using LT4/LT3 combination treatment for hypothyroid patients with persisting symptoms, despite variance with guideline recommendations.
