Serum Levels of α-Fetoprotein Increased More Than 10 Years Before Detection of Hepatocellular Carcinoma

David M. Hughes et al.

Summary: This study developed and validated a model to assess the risk of hepatocellular carcinoma (HCC) in patients with chronic liver disease based on longitudinal analysis of serum alpha-fetoprotein (AFP) measurements. The model showed good performance in identifying patients who developed HCC, allowing for classification into high-risk and low-risk groups. This model could help in selecting patients for surveillance and improving detection of HCC.

A Novel Blood-Based Panel of Methylated DNA and Protein Markers for Detection of Early-Stage Hepatocellular Carcinoma

Naga P. Chalasani et al.

Summary: A novel blood-based biomarker panel was developed for early-stage HCC detection, demonstrating high sensitivity. The panel outperformed other blood-based biomarkers, showing potential for clinically benefiting at-risk patients through liquid biopsy detection of early-stage HCC.

Mutations in circulating tumor DNA predict primary resistance to systemic therapies in advanced hepatocellular carcinoma

Johann von Felden et al.

Summary: This study reveals the mutational landscape of advanced HCC and identifies predictive biomarkers for response to systemic therapies. Mutations in the PI3K/MTOR pathway are associated with shorter PFS after tyrosine kinase inhibitor treatment, while WNT pathway mutations are not predictive of response after CPI therapy.

We need stronger evidence for (or against) hepatocellular carcinoma surveillance

Peter Jepsen et al.

Summary: Current guidelines recommend surveillance for HCC in cirrhosis patients, but actual compliance is lower than expected. The necessity and feasibility of a randomised trial comparing surveillance vs. no surveillance is argued, with the belief that participation in HCC surveillance would improve if based on trial results.

Liquid biopsy enters the clinic - implementation issues and future challenges

Michail Ignatiadis et al.

Summary: The approval of liquid biopsy assays for detecting genetic alterations in plasma cell-free DNA has marked a tipping point for the clinical use of this technology, especially in patients with advanced-stage cancer. The next frontier is likely to be the systemic treatment of patients with 'ctDNA relapse' and other potential future applications include cancer screening and diagnosis.

NGS-based accurate and efficient detection of circulating cell-free mitochondrial DNA in cancer patients

Yang Liu et al.

Summary: Mitochondrial DNA mutations play a role in the development of cancer, leading to the use of circulating cell-free mtDNA as a potential tumor biomarker. A new approach for profiling ccf-mtDNA mutations was developed based on systematic optimization of sequencing methodology. The study also highlighted the importance of proper processing of plasma samples for accurate detection of tumor-specific mtDNA mutations in cancer patients.

Hepatocellular carcinoma

Josep M. Llovet et al.

Summary: Liver cancer, particularly hepatocellular carcinoma (HCC), poses a significant global health challenge with rising incidence projected to exceed 1 million cases by 2025. Infection by hepatitis B and C viruses remains a major risk factor for HCC, while non-alcoholic steatohepatitis is emerging as a more common risk factor in Western countries. Advances in systemic therapies for HCC, including immunotherapies and targeted therapies, are expected to revolutionize the management of this disease.

Joshua D. Cohen et al.

Summary: The SaferSeqS method efficiently detects low-frequency mutations by introducing identical molecular barcodes and enriching target sequences with strand-specific PCR, achieving high sensitivity and specificity, and detecting variants at frequencies below 1 in 100,000 DNA template molecules with a background mutation rate of <5 x 10(-7) mutants per base pair (bp).

Epigenetics, fragmentomics, and topology of cell-free DNA in liquid biopsies

Y. M. Dennis Lo et al.

Summary: Liquid biopsies, which analyze cell-free DNA in blood plasma, are utilized for noninvasive prenatal testing, oncology, and organ transplant monitoring. The physical and molecular features of cell-free DNA fragments, along with their distribution over the genome, contain information about their tissues of origin. These features, including DNA methylation patterns and nucleosomal organization, hold promise for noninvasive detection of tissue-specific pathologies with a variety of clinical applications.

A comprehensive characterization of the cell-free transcriptome reveals tissue- and subtype-specific biomarkers for cancer detection

Matthew H. Larson et al.

Summary: Cell-free RNA is a promising analyte for cancer detection, with the potential to identify tissue- and subtype-specific biomarkers in breast and lung cancer patients. The presence of dark channel biomarker (DCB) genes in cfRNA from individuals with cancer suggests they could enhance cancer detection in patients with low levels of circulating tumor DNA.

Dynamic α-Fetoprotein Response and Outcomes After Liver Transplant for Hepatocellular Carcinoma

Karim J. Halazun et al.

Summary: This study validates the importance of AFP-R in selecting patients with HCC for liver transplant, showing its accurate prediction of recurrence and overall survival. AFP-R is a truly objective pre-transplant biological characteristic, and its incorporation into selection criteria allows for safe expansion of existing models and offers transplant opportunities to patients with acceptable tumor characteristics who might otherwise be denied potential cure.
