Prevalence, awareness, treatment and control of hypertension in Greece: EMENO national epidemiological study

Monotherapy vs combination treatments of different complexity: a meta-analysis of blood pressure lowering randomized outcome trials

Summary: Using drug combinations for hypertension treatment is recommended for most patients as they show better blood pressure-lowering effects compared to monotherapy. Randomized clinical trials indicate that combination treatments reduce fatal and nonfatal outcomes compared to simpler treatment regimens, however, the presence of treatment-related adverse events may affect the overall benefit of combination therapy.

Adherence to Single-Pill Versus Free-Equivalent Combination Therapy in Hypertension A Systematic Review and Meta-Analysis

Summary: Single-pill combination (SPC) therapy was found to lead to improved adherence and persistence compared to free-equivalent combination (FEC) therapy in patients with hypertension, resulting in better blood pressure control. Studies showed that patients receiving SPCs had significantly enhanced adherence and lower likelihood of discontinuation compared to those receiving FECs. Moreover, SPC therapy led to significant reductions in both systolic and diastolic blood pressure at week 12, highlighting its potential benefits for patients with hypertension.

Summary: The SIMPLIFY study identified therapeutic inertia in hypertension management, with key findings including patients with an average age of 64 years, average hypertension treatment duration of 8 years, nearly half of patients on monotherapy, over 80% of patients having their treatment modified during the study, and a preference for switching to single pill combination therapy. The study suggests that key drivers of therapeutic inertia are patient age, mild grade hypertension, isolated systolic hypertension, longer duration of antihypertensive treatment, and better therapeutic adherence.
