Exosomes, small extracellular vesicles released by cells, have been explored as a promising delivery vehicle due to their natural role in cell communication and biocompatibility. This review discusses exosome biogenesis, contents, isolation methods, and their natural functions, as well as different approaches for encapsulating therapeutic payloads. The use of molecules to improve pharmacokinetics, tumor homing-ligands, and stimuli-responsive elements to enhance cell specificity are also discussed. Finally, the current status of clinical-grade exosome-based therapies is outlined.
