The Role of Immune Checkpoint Molecules for Relapse After Allogeneic Hematopoietic Cell Transplantation

Natalie Kohler et al.

Summary: Immune checkpoint molecules play a crucial role in relapse and treatment after allo-HCT by inhibiting the graft-versus-leukemia effect, allowing tumor cells to escape. The activation of inhibitory receptors on T cell surfaces by immune checkpoint molecules facilitates immune escape. Research shows that immune checkpoint blockade holds potential value in improving the immunotherapeutic effect of relapse after allo-HCT.

Effect of Early Post-Transplantation Tacrolimus Concentration on the Risk of Acute Graft-Versus-Host Disease in Allogenic Stem Cell Transplantation

Nidhi Sharma et al.

Summary: Allogeneic hematopoietic stem cell transplantation is a potentially curative treatment for various hematological malignancies and disorders, but is often complicated by graft-versus-host disease and infectious complications. Tacrolimus is commonly used for GVHD prophylaxis, but the impact of early Tacrolimus levels on aGVHD is not well understood. Achieving optimal Tacrolimus levels early post-transplant may help mitigate the risk of aGVHD, but higher levels beyond the first week may lead to suppressed graft versus tumor effect and higher relapse rates.

Lymphopenia-induced lymphoproliferation drives activation of naive T cells and expansion of regulatory populations

S. Eldershaw et al.

Summary: Phenotypic and transcriptional analysis of T cells undergoing LIP after myeloablative autograft stem cell transplantation revealed a human LIP response characterized by cytokine and TCR-driven proliferation, leading to global T cell activation and preferential regulatory cell expansion, which may limit tumor-specific immunity. This highlights potential therapeutic opportunities for manipulating immunotherapy regimens incorporating LIP conditioning protocols.
