Patients' conceptions of undergoing physiotherapy for persistent low back pain delivered in Finnish primary healthcare by physiotherapists who had participated in brief training in cognitive functional therapy

Riikka Holopainen et al.

Summary: This study aimed to explore the conceptions of patients with persistent low back pain regarding physiotherapy delivered in Finnish primary healthcare by physiotherapists who had received brief training in Cognitive Functional Therapy (CFT). The findings revealed significant variation in patients' conceptions, with some experiencing limited access to care and lack of social support. However, positive experiences of physiotherapy, good collaboration with the physiotherapist, wider social support, understanding of pain's multidimensional nature, and acquisition of self-management skills were reported as positive aspects of undergoing physiotherapy that may lead to positive treatment outcomes.

What are the back beliefs of NHS employees, and does manual handling training influence them

David Nolan et al.

Summary: This study found that NHS employees hold many outdated views on back pain, and manual handling training had a small negative effect by exacerbating negative beliefs regarding vulnerability of the back and the relationship between pain and injury.

Relationship between psychological factors and spinal motor behaviour in low back pain: a systematic review and meta-analysis

Guillaume Christe et al.

Summary: Higher levels of pain-related fear, catastrophizing, and depression are significantly associated with reduced spinal movement amplitudes and increased muscle activity in low back pain patients. Pain intensity has a minimal independent association with spinal motor behavior, and the small effect sizes found in the meta-analyses question the role of psychological factors as major causes of spinal movement avoidance.

Fear-avoidance beliefs are associated with reduced lumbar spine flexion during object lifting in pain-free adults

Deborah Knechtle et al.

Summary: There has been a long-held belief that activities like lifting with a flexed spine can be harmful for the back and lead to low back pain. Research shows that fear related to pain is associated with reduced range of motion in the lumbar spine. Recent evidence suggests that maintaining a protective trunk movement strategy could pose a risk for persistent low back pain.

What do patients value learning about pain? A mixed-methods survey on the relevance of target concepts after pain science education

Hayley B. Leake et al.

Summary: The study examined the pain concepts valued by individuals with persistent pain who participated in pain science education. Findings revealed that patients placed importance on understanding that pain does not necessarily indicate bodily damage, recognizing the multifactorial influences on pain, and being able to retrain an overprotective pain system to lessen pain. This patient-centered approach offers valuable insights for refining pain education interventions.

How does physical examination findings influence physiotherapists' decision-making when matching treatment to patients with low back pain?

Birgitta Widerstrom et al.

Summary: The study found that physiotherapists choose different treatment approaches for LBP based on symptoms and signs, but sometimes decisions are also influenced by personal biases unrelated to clinical presentation. The results emphasize the need for clinicians to balance multiple knowledge sources and be aware of their preconceptions for meaningful clinical decision-making.

Physiotherapists' attitudes and beliefs about low back pain influence their clinical decisions and advice

Guillaume Christe et al.

Summary: The study found that helpful beliefs were generally present among physiotherapists, but unhelpful beliefs related to back protection and the special nature of low back pain were frequently identified. Individual characteristics explained 17% of the Back-PAQ score, and unhelpful beliefs were associated with clinical decisions towards back protection and movement avoidance.
