External Validation of Three Available Grading Systems for Medullary Thyroid Carcinoma in a Single Institution Cohort

Summary: Medullary thyroid carcinoma (MTC) is a rare thyroid carcinoma with variable clinical behavior. A new grading system called International Medullary Thyroid Carcinoma Grading System (IMTCGS) has been proposed to define high- or low-grade MTC based on proliferative activity and necrosis. In a validation cohort, the IMTCGS was found to have a significant prognostic impact on disease recurrence and disease-free survival. However, none of the grading systems could predict overall survival.

Overview of the 2022 WHO Classification of Neuroendocrine Neoplasms

Summary: In this review, the changes and features of neuroendocrine neoplasms (NENs) in the 2022 WHO Classification of Endocrine and Neuroendocrine Tumors are detailed. The consolidation of nomenclature distinguishing different neoplasms and the importance of Ki67 as a tool for classification and grading are emphasized. The clinical relevance of proper classification and the tools available to pathologists for accurate classification are explained. The article also discusses the differential diagnoses and theranostic biomarkers for NENs, as well as the role of pathologists in identifying precursor lesions and applying molecular immunohistochemistry.

Grading of Medullary Thyroid Carcinoma: an Interobserver Reproducibility Study

Summary: Grade in medullary thyroid carcinoma (MTC) has been shown to be prognostic. This study evaluated the reproducibility of assessed grade in MTC and found that the International Medullary Thyroid Carcinoma Grading System (IMTCGS) is both prognostic and reproducible.

Summary: Amphicrine phenotype in medullary thyroid carcinoma (MTC) is a rare phenomenon characterized by tumor cells that show both endocrine and exocrine differentiation. This report discusses the cytological, histopathological, and ultrastructural features of amphicrine MTC, as well as radiological and novel MUC immunohistochemistry findings from a case of MEN2B syndrome patient. This rare subtype of MTC is associated with MEN2B syndrome and biological aggressiveness, making it of clinical interest.

Metastatic Risk Stratification of 2526 Medullary Thyroid Carcinoma Patients: A Study Based on Surveillance, Epidemiology, and End Results Database

Summary: This study developed a method to stratify the risk of metastasis at the initial presentation of medullary thyroid carcinoma (MTC) patients. By collecting data from a large population and selecting informative variables, the researchers developed a risk table that showed good performance in predicting metastatic risk. The incorporation of this table into the International MTC Grading System requires further clinicopathological studies.
