Bloodstream infections in patients with rectal colonization by Klebsiella pneumoniae producing different type of carbapenemases: a prospective, cohort study (CHIMERA study)

Marco Falcone et al.

Summary: The study found that NDM-Kp is associated with a higher risk of BSI compared to KPC-Kp, and this association seems to be strongly related to the high-risk clone ST147.

Infections in Patients Colonized With Extended-spectrum Beta-Lactamase-Producing Enterobacterales: A Retrospective Cohort Study

Summary: Majority of infections in hospitalized patients colonized with ESBL-PE are not caused by ESBL-PE. Site of infection and patient-level exposures may serve as useful predictors for nESBI,-PE infections, potentially guiding empiric treatment recommendations.

Oral decontamination with colistin plus neomycin in solid organ transplant recipients colonized by multidrug-resistant Enterobacterales: a multicentre, randomized, controlled, open-label, parallel-group clinical trial

Maria Carmen Farinas et al.

Summary: The study found that oral colistin-neomycin did not reduce MDR-E infections in SOT recipients. Colistin resistance and adverse effects like diarrhea were noted as potential issues during the treatment.

Colonization with multidrug-resistant organisms is associated with in increased mortality in liver transplant candidates

Philip G. Ferstl et al.

Summary: The prevalence of multidrug-resistant organisms is a major health concern for liver cirrhosis patients. This study found that MDRO colonization in liver transplant candidates and recipients is associated with increased mortality on the waiting list, especially with CRGN colonization.

Infectious Diseases Society of America Guidance on the Treatment of Extended-Spectrum β-lactamase Producing Enterobacterales (ESBL-E), Carbapenem-Resistant Enterobacterales (CRE), and Pseudomonas aeruginosa with Difficult-to-Treat Resistance (DTR-P. aeruginosa)

Pranita D. Tamma et al.

Summary: This guidance document provides recommendations for the treatment of antimicrobial-resistant infections commonly encountered in US hospitals, focusing on ESBL-E, CRE, and DTR-P. aeruginosa. It includes preferred and alternative treatment recommendations, discussion on empiric treatment selection and management considerations, and will be periodically updated. The current version is as of 17 September 2020, with plans to expand recommendations to include other problematic gram-negative pathogens in future versions.

Relative faecal abundance of extended-spectrum β-lactamase-producing Enterobacterales and its impact on infections among intensive care unit patients: a pilot study

B. Pilmis et al.

Summary: This pilot study assessed the association between faecal ESBL-PE abundance and ESBL-PE related infections, finding that patients exposed to antibiotics had higher ESBL-PE relative abundance, and patients colonized with non-E. coli strains had higher ESBL relative abundance. Patients with concordant ESBL-PE colonization had significantly higher ESBLPE-RA than discordant patients.
