Increasing operator experience and transitioning to retrograde approach when antegrade technique fails have improved success rates in CTO PCI. The upfront retrograde approach is used in some cases with moderate success, and the presence of usable collateral is the most important predictor. While pursuing improved revascularization rates, patient safety and complications rates should also be considered.
