A prospective observational study of persistent opioid use after complex foot and ankle surgery

Summary: The study found that patients who regularly used opioids before surgery had a significantly higher probability of continuing opioid use six months after surgery. Preoperative opioid use was positively correlated with persistent postoperative opioid use.

Opioid prescribing and utilization patterns in patients having elective hip and knee arthroplasty: association between prescription patterns and opioid consumption

Summary: This study demonstrated that the amount of prescribed opioids is positively related to opioid consumption. Majority of unused opioids are from patients who used opioids for two days or less. Patient satisfaction remains high regardless of the amount of prescribed opioids.

Evaluation of a quality improvement bundle aimed to reduce opioid prescriptions after Cesarean delivery: an interrupted time series study

Summary: After implementing a quality improvement bundle, opioid prescriptions at discharge following Cesarean delivery significantly decreased at Mount Sinai Hospital in Toronto. This decrease was sustained over the study period, and there were no visits to the postnatal assessment clinic for inadequate pain control.

Variability in discharge opioid prescribing practices for children: a historical cohort study

Summary: Canadian seniors undergoing hip and knee replacement surgery often require opioids postoperatively for pain relief, with those living in long-term care facilities being more likely to use opioids chronically. Female sex and comorbidities are also risk factors for prolonged opioid use following joint replacement surgery.

Trends in postoperative opioid prescribing in Ontario between 2013 and 2019: a population-based cohort study

Summary: The proportion of patients filling an opioid prescription postoperatively remained stable during the study period, but the total morphine milligram equivalent (MME) dose decreased after 2016. Opioid prescribing remained significantly higher than available recommendations, especially for low pain procedures.

Elective surgery in ankle and foot disorders-best practices for management of pain: a guideline for clinicians

Summary: This study aimed to develop best practice pain management guidelines for patients undergoing complex elective foot and ankle surgery. A multidisciplinary expert panel was formed and a Delphi process was used to reach consensus on pain management recommendations. Strong consensus was achieved in many areas, indicating considerable agreement among experts despite limited evidence. However, there was no consensus on important issues related to opioid prescribing and cessation, highlighting the need for further research to determine best practices.

The results show a decrease in the opioid dispensation rate after Cesarean delivery, though the decrease has been slow, and there are variations among different hospitals.
