Analgesia for major laparoscopic abdominal surgery: a randomised feasibility trial using intrathecal morphine

The ALLEGRO trial: a placebo controlled randomised trial of intravenous lidocaine in accelerating gastrointestinal recovery after colorectal surgery

Summary: The ALLEGRO trial aims to assess the effectiveness of perioperative intravenous lidocaine in improving return of GI function after minimally invasive colorectal surgery. The study suggests that IV lidocaine can accelerate the recovery of GI function, reduce the rate of postoperative ileus, shorten hospital stay, alleviate pain and discomfort, and lead to faster recovery and discharge from hospital, resulting in significant cost savings for the National Health Service (NHS).

Epidural vs. transversus abdominis plane block for abdominal surgery - a systematic review, meta-analysis and trial sequential analysis

Summary: Epidural analgesia showed lower pain scores at rest at 12 hours compared to transversus abdominis plane block, but the quality of evidence was low. However, there was no difference at 24 hours. Epidural analgesia reduced the need for intravenous morphine-equivalent consumption, but also increased the risk of hypotension. Confidence in the evidence varied, creating uncertainty in the estimates when choosing between epidural analgesia and transversus abdominis plane block.

Erector spinae plane block: A narrative review with systematic analysis of the evidence pertaining to clinical indications and alternative truncal blocks

Summary: This narrative review discusses the anatomy, mechanism of action, techniques, pharmacology, indications, complications and substitutes for erector spinae plane (ESP) blocks. ESP blocks are favored over intercostal blocks for providing analgesia, and future trials are needed to investigate optimal technique and dosage of local anesthetic. Comparisons with other truncal blocks are also recommended for further investigation.

Summary: The study found that QLB can reduce postoperative opioid consumption, prolong the time to the first rescue opioid analgesic, and diminish the incidence of postoperative nausea and vomiting. In addition, QLB is shown to be a feasible option for postoperative analgesia after abdominal and hip surgery. The study also showed that complications and adverse effects of QLB were minimal.
