Standardization of EUS imaging and reporting in high-risk individuals of pancreatic adenocarcinoma: consensus statement of the Pancreatic Cancer Early Detection Consortium

Tamas A. Gonda et al.

Summary: The adoption of a standardized EUS reporting template will improve consistency in clinical decision-making for individuals at risk of pancreatic cancer and provide complete and accurate reporting of pancreatic abnormalities. Standardization will also facilitate research and clinical trial design, allowing for comparison of results across different centers.

Pancreatic Cystic Lesions and Malignancy: Assessment, Guidelines, and the Field Defect

Frank H. Miller et al.

Summary: The detection and diagnosis of pancreatic cystic lesions, ranging from benign to invasive cancer, can be challenging, often requiring expert consensus, surveillance and updated guidelines. Features associated with increased malignancy risk are crucial for proper management, and stakeholders must determine which guidelines to follow based on individual institutions. Expert radiologists play a critical role in detection, characterization, and follow-up recommendations for these lesions, as well as in the detection of associated cancer.

Long-term yield of pancreatic cancer surveillance in high-risk individuals

Kasper A. Overbeek et al.

Summary: The study aimed to determine the long-term yield of pancreatic cancer surveillance in hereditary high-risk individuals. Results showed substantial diagnostic yield in high-risk mutation carriers, but non-existent in mutation-negative familial pancreatic cancer (FPC) kindreds. Challenges in timely identification of resectable lesions were encountered despite concurrent use of EUS and MRI/MRCP, with the need for more sensitive diagnostic markers, including biomarkers, highlighted.

Summary: The study demonstrates that an ultrashort-protocol MRI provides nearly identical information to more expensive longer protocols in monitoring IPMN patients, confirming its effectiveness in identifying cystic mural nodules and key features.

Adding Delayed Phase Images to Dual-Phase Contrast-Enhanced CT Increases Sensitivity for Small Pancreatic Ductal Adenocarcinoma

Summary: The addition of a delayed phase to pancreatic and portal venous phase CT increases sensitivity for small PDAC without loss of specificity, partly related to delayed phase hyperattenuation of some small PDACs showing pancreatic phase isoattenuation.

CT Abnormalities of the Pancreas Associated With the Subsequent Diagnosis of Clinical Stage I Pancreatic Ductal Adenocarcinoma More Than 1 Year Later: A Case-Control Study

Fumihito Toshima et al.

Summary: The study aimed to investigate findings of earlier diagnosis of pancreatic ductal adenocarcinoma (PDAC) on CT examinations, and found a higher frequency of focal pancreatic abnormalities on prediagnostic CT in PDAC patients. Atrophy and MPD change were common abnormalities in PDAC patients, with a relatively shorter time from their appearance to diagnosis in PDAC patients.

Intraductal papillary mucinous neoplasm (IPMN) of the pancreas: recommendations for Standardized Imaging and Reporting from the Society of Abdominal Radiology IPMN disease focused panel

Elizabeth M. Hecht et al.

Summary: There are numerous publications detailing the imaging features, management, and follow-up recommendations for malignant transformation of intraductal papillary mucinous neoplasms (IPMN). However, practical aspects of imaging IPMN still lack consensus, including how to measure, define reporting terminology, standardize reporting, and unify surveillance guidelines. The Society of Abdominal Radiology (SAR) established Disease-Focused Panels (DFP) to address current controversies and limitations in imaging pancreatic IPMN. The goal is to provide a practical review of key imaging characteristics of IPMN for radiologists to improve patient care, education, and research.
