Modified Duke/European Society of Cardiology 2015 clinical criteria for infective endocarditis: time for an update?

Echocardiographic features of infective endocarditis in South Africa: A prospective cohort study

Summary: This study observed a decrease in the prevalence of underlying structural valve abnormalities among patients with infective endocarditis (IE) in South Africa. The viridans group of streptococci was an uncommon cause of IE in patients without underlying valvular abnormality. The presence of a predisposing endocardial abnormality was not associated with an increased risk of in-hospital mortality or embolic events, but linear vegetation length >10 mm was associated with an increased risk of in-hospital mortality in patients with left-sided IE.

Summary: The World Heart Federation (WHF) screening criteria lack a strict definition of anterior mitral valve leaflet (AMVL) restriction. Using a novel definition, two distinct AMVL restriction configurations were identified, with distal tip AMVL restriction associated with rheumatic heart disease (RHD) features, while gradual bowing AMVL restriction appears to be a normal, benign variant. Further investigation is needed to explore the potential of gradual bowing AMVL restriction as a rule-out test for RHD.

Summary: The introduction of a systematic approach to organism detection and management by an endocarditis team can help accurately identify the causes of infective endocarditis and reduce in-hospital mortality in patients with blood culture negative infective endocarditis (BCNIE). Staphylococcus aureus was found to be the most common cause of IE, with non-culturable organisms like Bartonella quintana identified as important causes of BCNIE.
