The Impact of the COVID-19 Pandemic on Drug Use Behaviors, Fentanyl Exposure, and Harm Reduction Service Support among People Who Use Drugs in Rural Settings

Rebecca S. Bolinski et al.

Summary: The COVID-19 pandemic has exacerbated the opioid overdose crisis in the US, particularly in rural areas. Our study found that drug availability changed, overdose risk increased, and other drug-related harms were faced by people who use drugs in rural areas during the pandemic. It is important to ensure access to harm reduction services, including overdose prevention and drug checking for this vulnerable population.

PrEP Care Continuum Engagement Among Persons Who Inject Drugs: Rural and Urban Differences in Stigma and Social Infrastructure

Suzan M. Walters et al.

Summary: The study found that stigma is a major barrier to PrEP care engagement among people who inject drugs. Awareness and knowledge levels differ between urban and rural environments, but willingness to use PrEP is similar. Stigmas against drug use, HIV, and sexualities were identified as the main barriers to PrEP uptake in rural areas.

Summary: This article examines the impact of restrictive take-home dosing policies in Methadone Maintenance Treatment (MMT) on patients, finding that most patients are critical of limited take-home dose access. These policies lead to issues with meeting daily responsibilities and contribute to a sense of cruelty and stigma among patients.

Summary: The study found that stigma against nonmedically using drugs in rural areas is a significant issue, particularly manifested in the attitudes of professionals in law enforcement, healthcare, and social services. Most respondents, including nonmedically using drug users, perceived their communities as lacking awareness or being dismissive towards drug use and harm reduction services.

Acceptability and Effectiveness of Hepatitis C Care at Syringe Service Programs for People Who Inject Drugs in New York City

Brandon Muncan et al.

Summary: The study found that people who inject drugs (PWID) generally perceive local syringe service programs (SSPs) as accessible and effective sites for hepatitis C (HCV) care. Key themes identified include the non-stigmatizing environment of SSPs, the role of SSPs in improving HCV knowledge, and the acceptability of SSPs for HCV care among PWID. This highlights the importance of understanding PWID attitudes towards SSPs in developing targeted strategies to reduce HCV transmission and achieve elimination goals.

The rise of illicit fentanyls, stimulants and the fourth wave of the opioid overdose crisis

Daniel Ciccarone

Summary: The review provides an update on the rising trend of illicit fentanyls, overdose risks, and treatment options, particularly focusing on the increasing use of synthetic opioids like fentanyl and its analogs in the US. Methamphetamine and cocaine-driven stimulant-related overdoses are also on the rise nationwide, contributing to the 'fourth wave' of high mortality involving these substances. The use of illicit fentanyls remains a major contributor to overdose deaths, and the increase in stimulant-related deaths seems to be connected to the ongoing opioid epidemic.

How urban and rural built environments influence the health attitudes and behaviors of people who use drugs

Jerel M. Ezell et al.

Summary: Research suggests that the built environment is associated with drug use, but there is limited scholarship focusing on specific features that influence drug use behaviors. Qualitative interviews with individuals in urban and rural settings reveal that the built environment regulates drug use milieu, impacting individuals' social conditions and resource access. Social and economic inequities related to housing instability are observed to influence drug use patterns in both settings.

Summary: Research has shown that most people who use drugs enter treatment under constrained conditions related to the drugs' illegality, influenced by factors like family pressure, fear of losing children, and internalized stigma. In such cases, treatment models need to be reconceptualized outside of normative narratives of individual choice to understand how larger structural constraints affect choices.
