Targeted next-generation sequencing of adult gliomas for retrospective prognostic evaluation and up-front diagnostics

J. K. Petersen et al.

Summary: The study successfully reclassified a population-based cohort of adult glioma patients based on WHO 2016 criteria, confirming three major prognostic subgroups. The feasibility of gene panel NGS in daily diagnostics was demonstrated, and it was found that molecular features of IDH-wildtype diffuse glioma were linked to poor survival similar to IDH-wildtype glioblastoma patients. The prognostic significance of distinguishing between WHO grade II and III in IDH-mutant diffuse astrocytic gliomas was also retained.

TERT Promoter Mutation Analysis for Blood-Based Diagnosis and Monitoring of Gliomas

Koushik Muralidharan et al.

Summary: Liquid biopsy offers a minimally invasive tool to diagnose and monitor the heterogeneous molecular landscape of tumors. This study developed a novel detection technique for TERT promoter mutations in glioma patients, demonstrating clinically relevant sensitivity and specificity. Results show that peripheral TERT-mutant allele frequency reflects the clinical course of the disease.

TERT Promoter Alterations in Glioblastoma: A Systematic Review

Nathalie Olympios et al.

Summary: Glioblastoma is the most common malignant primary brain tumor in adults, with an incidence of 2 to 3 cases per 100,000 persons in North America and Europe. Alterations in the telomerase reverse transcriptase promoter, highly recurrent in all glioblastomas, including both IDH wild type and mutated, are potential mechanisms enabling cancer growth.

Chun Zeng et al.

Summary: The study investigated the genetic landscape of Chinese glioma patients using multigene next-generation sequencing, revealing somatic mutations in the majority of cases, amplification of genes associated with glioblastoma, and significant genomic profile differences between different pathological subtypes and WHO grades. Compared to a database primarily of Caucasians, Chinese glioma patients showed higher mutation rates in H3F3A mutations and MET amplifications, and lower rates in TERT mutations and EGFR and CDKN2A/B copy number variations, highlighting the need for tailored treatment options in different racial/ethnic groups.

Quantitative MRI-based radiomics for noninvasively predicting molecular subtypes and survival in glioma patients

Jing Yan et al.

Summary: MRI-based radiomics can be used for noninvasively detecting molecular groups and predicting survival in gliomas, regardless of grades. The image fusion model achieved high accuracy in predicting IDH and TERT status, while the cT1WI-based radiomic signature alone performed well in predicting 1p/19q status. Predictive molecular groups showed comparable prognostic value to actual ones in predicting PFS and OS. Prognostic nomograms based on grades and predictive molecular groups yielded good predictive performance for PFS and OS.
