Chronic transplant glomerulopathy: New insights into pathogenesis

Avantee Gokhale et al.

Summary: There have been significant advances in short-term outcomes in renal transplantation, but long-term allograft survival remains a challenge, especially with the complication of transplant glomerulopathy (TG). Novel mechanistic insights into the pathogenesis of TG have been discussed, highlighting the roles of cell-mediated immunity and podocyte stress, as well as the lack of clinical trial data for managing this prevalent condition in renal transplants.

New 'Antigens' in Membranous Nephropathy

Sanjeev Sethi

Summary: Membranous nephropathy is a disease caused by immune complex deposition along the glomerular basement membrane, with PLA2R and THSD7A being the main antigens. New mass spectrometry techniques have led to the discovery of four novel types of MN, each with distinct clinical and pathologic features. Additional studies are needed to understand the pathophysiology, treatment response, and outcomes of these new MN subtypes.

A Randomized Clinical Trial of Anti-IL-6 Antibody Clazakizumab in Late Antibody-Mediated Kidney Transplant Rejection

Konstantin Doberer et al.

Summary: This study was a phase 2 randomized trial evaluating the safety and efficacy of the anti-IL-6 antibody clazakizumab in late antibody-mediated rejection (ABMR) post-transplantation. Results showed that patients receiving clazakizumab had decreased donor-specific antibodies and modulated rejection-related gene-expression patterns.

Risk factors, histopathological features, and graft outcome of transplant glomerulopathy in the absence of donor-specific HLA antibodies

Aleksandar Senev et al.

Summary: Transplant glomerulopathy can occur independently of HLA-DSA, with less inflammation and better graft survival compared to cases with HLA-DSA. The presence of HLA-DSA was identified as a risk factor for glomerulopathy development, and HLA-DSA negative cases had a lower risk of graft failure compared to HLA-DSA positive cases. Landmark analysis showed that HLA-DSA negative patients with glomerulopathy still had a significant increased risk of graft failure, indicating the importance of monitoring and managing transplant glomerulopathy even in the absence of HLA-DSA.
