Management of Descending Necrotizing Mediastinitis, a Severe Complication of Deep Neck Infection, Based on Multidisciplinary Approaches and Departmental Co-Ordination

Chia-Ying Ho et al.

Summary: This study presents the outcomes of multidisciplinary approaches for treating descending necrotizing mediastinitis (DNM) originating from deep neck infection (DNI). The results show that multidisciplinary treatment, early comprehensive medical care, and coordination among departments significantly reduce mortality. Intensive interventions are required for patients with severe inflammation and high levels of C-reactive protein (CRP).

Factors Affecting Patients with Concurrent Deep Neck Infection and Acute Epiglottitis

Chia-Ying Ho et al.

Summary: This study aimed to investigate the risk factors in patients with concurrent deep neck infection (DNI) and acute epiglottitis (AE). The study found that involvement of the parapharyngeal space and involvement of the submandibular space were independent risk factors for patients with concurrent DNI and AE.

Factors Affecting Patients with Concurrent Deep Neck Infection and Cervical Necrotizing Fasciitis

Shih-Lung Chen et al.

Summary: Higher levels of CRP and blood sugar were identified as independent risk factors for the concurrence of DNI and CNF. In terms of prognosis, significant differences were observed in the length of hospital stay and therapeutic management between patients with and without CNF. While there were no significant differences in pathogens, patients with DNI alone had a higher rate of specific pathogen non-growth compared to those with concurrent DNI and CNF.

Parotid Space, a Different Space from Other Deep Neck Infection Spaces

Shih-Lung Chen et al.

Summary: This study identified significant differences in clinical presentation, laboratory findings, microbial culture results, and treatment approaches between parotid space infections and other deep neck space infections. Parotid space infections were more often associated with Klebsiella pneumoniae.

Factors Affecting the Necessity of Tracheostomy in Patients with Deep Neck Infection

Shih-Lung Chen et al.

Summary: The need for tracheostomy in patients with DNI is associated with older age, involvement of multiple spaces, and the presence of mediastinitis. The parapharyngeal and retropharyngeal spaces are commonly involved in DNI patients requiring tracheostomy, with Streptococcus constellatus being the most common pathogen.
