Septic Failure After One-Stage Exchange for Prosthetic Joint Infection of the Hip: Microbiological Implications

Risk Factors Associated With Nephrotoxicity During Outpatient Intravenous Vancomycin Administration

Summary: Outpatient administration of vancomycin is associated with a high incidence of nephrotoxicity. Factors such as longer duration of therapy, higher maximum trough concentration, co-administration of a fluoroquinolone or metronidazole, and other potentially nephrotoxic medications were found to increase the risk of acute kidney injury (AKI). Co-administration of a fluoroquinolone, any nephrotoxic medication, and a higher maximum vancomycin trough were all significant risk factors for AKI development in this cohort. Further research is needed to assess the impact on long-term clinical outcomes and explore measures to reduce nephrotoxicity risk.

Ninety-Day Follow-up Is Inadequate for Diagnosis of Fracture-related Infections in Patients with Open Fractures

Summary: In patients with open fractures, the median time for the onset of infection is 52 days. Follow-up of 90 days only captures 64% of infections, while extending it to 1 year captures 89% of infections. It is recommended to have a follow-up of at least 1 year, especially after fracture union. It is still possible for a small percentage of patients to develop infections beyond the first year after the management of an open fracture. Future studies should consider extending the follow-up period to clarify this issue.
