Challenges in Implementing Cardiovascular Risk Scores for Assessment of Young People With Childhood-Onset Autoimmune Rheumatic Conditions

Coziana Ciurtin et al.

Summary: Cardiovascular risk stratification tools are important in clinical practice for managing primary prevention of cardiovascular disease. However, there is limited knowledge about how to accurately estimate cardiovascular risk in children and adolescents, as well as the reliability of the risk stratification tools validated in adult populations. Chronic inflammation associated with autoimmune rheumatic disease increases the risk of atherosclerosis in patients of all ages. Young people with childhood-onset autoimmune rheumatic diseases have a higher cardiovascular risk compared to healthy controls of the same age. Further research is needed to address the need for accurate identification and management of cardiovascular risk in young people, especially those with underlying chronic inflammation.

Lipid metabolism in autoimmune rheumatic disease: implications for modern and conventional therapies

George Robinson et al.

Summary: Conventional therapies for autoimmune rheumatic diseases can have unpredictable effects on cell metabolism, including lipid metabolism. New therapeutic technologies offer more targeted approaches to reduce inflammation and prevent off-target metabolic consequences. Lipid metabolism plays a key role in inflammation and the mechanisms of action of AIRD therapeutics. Co-therapies targeting lipid metabolism have the potential to reduce immunometabolic complications and cardiovascular disease risk in AIRD patients.

Menopausal Hormone Therapy and Cardiovascular Disease: The Role of Formulation, Dose, and Route of Delivery

Chrisandra L. Shufelt et al.

Summary: Menopausal hormone therapy has varying effects on cardiometabolic markers and cardiovascular disease risk depending on formulation, dose, and route of delivery. Oral estrogens without progestogens have positive effects on lipoprotein levels, glycemia, insulin, and CVD risk, while adding progestogens blunts the lipid-related effects. Transdermal estrogens have less impact on coagulation, inflammation, and lipids compared to oral estrogens, potentially posing a lower risk of venous thromboembolism and stroke.

Summary: The cardiovascular benefit and safety of treating low testosterone conditions is debated due to lack of adequate studies on testosterone replacement therapy's impact on CV risk. Clinicians should carefully consider the use of testosterone in men with atherosclerotic cardiovascular disease until more research is available.

Sex Hormones and Gender Influence the Expression of Markers of Regulatory T Cells in SLE Patients

Ram P. Singh et al.

Summary: Gender and sex hormones can influence susceptibility to SLE by affecting regulatory T cells and FoxP3 expression, with females being more susceptible than males. Female SLE patients have higher estradiol levels and lower testosterone levels, which may lead to reduced T-regulatory cells and decreased FoxP3 expression, contributing to the increased susceptibility to SLE and other autoimmune diseases.

The Lancet women and cardiovascular disease Commission: reducing the global burden by 2030

Birgit Vogel et al.

Summary: Despite decades of grassroots campaigns to raise awareness about cardiovascular disease in women, there has been stagnation in reducing the overall burden of the disease in the past decade. This Commission aims to fill knowledge gaps and provide recommendations to reduce the global burden of cardiovascular disease in women by 2030, with a focus on raising awareness of gender-related disparities.

Sex hormones drive changes in lipoprotein metabolism

George A. Robinson et al.

Summary: The study shows that sex hormones play a significant role in influencing lipid levels, with post-pubertal cis-women having higher levels of high-density lipoproteins (HDL). Cross-sex hormone treatment can induce similar effects in transgender individuals, indicating that sex hormones regulate lipid metabolism. This could contribute to the sexual dimorphism in cardiovascular disease risk observed post puberty, potentially informing sex-specific therapeutic strategies for CVD management.
