Genomic and clinical characterisation of multidrug-resistant carbapenemase-producing ST231 and ST16 Klebsiella pneumoniae isolates colonising patients at Siriraj hospital, Bangkok, Thailand from 2015 to 2017

Adhiratha Boonyasiri et al.

Summary: Infections caused by carbapenemase-producing Enterobacteriaceae (CPE) are a growing global public health threat, with significant mortality rates observed worldwide. This study focused on clinical data from patients with CPE infections, specifically Klebsiella pneumoniae isolates, analyzing antimicrobial resistance and virulence determinants to improve management strategies for CPE infections in hospitalised patients. The majority of CPE isolates were K. pneumoniae, with bla(OXA-232) and bla(NDM-1) being the most common carbapenemase genes identified. Phylogenetic analysis revealed the spread of carbapenemase-producing K. pneumoniae within the patient population, with certain subclades associated with higher virulence and specific virulence factors.

Co-infections of two carbapenemase-producing Enterobacter hormaechei clinical strains isolated from the same diabetes individual in China

Manlin Ding et al.

Summary: This study characterized two multi-drug resistant E. hormaechei isolates from a diabetes patient in Sichuan, China, revealing their resistance to various antibiotics, biofilm formation ability, and virulence. The presence of resistant genes in these strains poses a serious threat to global public health and highlights the importance of constant monitoring and prevention strategies.

Summary: In this study, a novel plasmid co-harboring bla(IMP-4) and bla(NDM-1) was characterized in Klebsiella michiganensis 7525, highlighting the complexity and fluctuating stability of plasmids in evolution. The study provides insights into the formation of plasmids through genetic rearrangement and emphasizes the importance of clinical infection control in response to carbapenem-resistant Enterobacteriaceae (CRE).

Summary: The dissemination of plasmid-mediated carbapenemases within Klebsiella oxytoca and Klebsiella grimontii differs, with more research on the former than the latter. Whole-genome sequencing revealed that K. grimontii strains carry various plasmids, some of which are associated with other Enterobacterales species.

A genomic surveillance framework and genotyping tool for Klebsiella pneumoniae and its related species complex

Margaret M. C. Lam et al.

Summary: Kleborate is a tool designed for analyzing genomes of Klebsiella pneumoniae and its related species complex, aimed at supporting genomic surveillance and epidemiology in research, clinical, and public health settings. By applying Kleborate to analyze Klebsiella genomes, global trends in antimicrobial resistance and virulence can be highlighted as examples of what could be achieved by using this genomic framework within more systematic genomic surveillance efforts.

IS26-mediated amplification of blaOXA-1 and blaCTX-M-15 with concurrent outer membrane porin disruption associated with de novo carbapenem resistance in a recurrent bacteraemia cohort

William C. Shropshire et al.

Summary: By analyzing genomic data and conducting serial passaging experiments on clinical ESBL-E isolates, this study revealed the IS26-mediated mechanisms of beta-lactamase gene amplification and outer membrane porin disruption driving the emergence of clinical non-CP-CRE. These amplifications were found to be stable in the absence of antimicrobial pressure, demonstrating the potential of long-read sequencing in identifying mobile genetic element mechanisms that contribute to antimicrobial resistance.

In Vitro Activity of Cefepime-Zidebactam, Ceftazidime-Avibactam, and Other Comparators against Clinical Isolates of Enterobacterales, Pseudomonas aeruginosa, and Acinetobacter baumannii: Results from China Antimicrobial Surveillance Network (CHINET) in 2018

Yang Yang et al.

Summary: The study found that cefepime-zidebactam exhibited potent activity against almost all Enterobacterales and good activity against P. aeruginosa, while ceftazidime-avibactam showed good in vitro activity against Enterobacterales and P. aeruginosa but less activity against Acinetobacter baumannii. Most of the carbapenem-resistant Enterobacteriaceae isolates were susceptible to cefepime-zidebactam and ceftazidime-avibactam showed good activity against bla(KPC-2)-positive Enterobacterales and carbapenem-resistant P. aeruginosa.
