Long-term safety and efficacy of patisiran for hereditary transthyretin-mediated amyloidosis with polyneuropathy: 12-month results of an open-label extension study

Summary: Hereditary transthyretin-mediated amyloidosis is a rare genetic disease caused by mutations in the TTR gene. Long-term treatment with patisiran, an RNA interference therapeutic, showed sustained efficacy and acceptable safety profile in patients with this condition, as demonstrated in a 12-month interim analysis of a global open-label extension study.

Summary: The study demonstrates that TTR can interfere with IAPP amyloid-formation, prolonging the lag-phase and impairing elongation. The interfering ability correlates inversely with the thermodynamic stability of TTR, but not with kinetic stability. TTR's interference ability remains effective in the low pH environment of IAPP-containing granules, but TTR-stabilizing drugs partially weaken its efficacy.

Summary: The aggregation of human islet amyloid polypeptide (IAPP) is associated with type II diabetes, with IAPP oligomers identified as the most cytotoxic species. Secondary nucleation is the main mechanism for generating new aggregates, with toxicity peaking at the maximum secondary nucleation rate. Compounds targeting IAPP secondary nucleation may be the most effective therapeutic candidates for type II diabetes.
