Pathomechanisms in hepatic encephalopathy

Summary: Hepatic encephalopathy (HE) is a common neuropsychiatric complication in patients with acute or chronic liver failure, characterized by disturbances in sensory, motor, and cognitive functions. HE is triggered by various factors including ammonia, benzodiazepines, proinflammatory cytokines, and hyponatremia. In patients with liver cirrhosis, HE induces functionally relevant alterations, impairing astrocyte/neuronal functions and communication, and leading to a global slowing of brain activity and networks. These changes are associated with increased cerebral ammonia, alterations in neurometabolite and neurotransmitter concentrations, and cortical excitability in HE patients.
