Incorporating radiomics into clinical trials: expert consensus on considerations for data-driven compared to biologically driven quantitative biomarkers

Laure Fournier et al.

Summary: Existing quantitative imaging biomarkers go through a well-understood validation process before being included in clinical trials, while radiomics extracts features from imaging data without prior assumptions. The selection and incorporation of relevant features in radiomics require additional considerations for meaningful imaging endpoints. The large number of radiomic features tested may lead to sample sizes impractical within clinical trials.

Test-retest repeatability of T1rho (T1ρ) MR imaging in the head and neck

Qi Yong H. Ai et al.

Summary: This study evaluated the repeatability of quantitative T1rho imaging in the head and neck region by performing scans on healthy participants on three occasions. The ICCs for normal tissues showed almost perfect repeatability for intra-session scans and substantial for inter-session scans. The T1rho values varied among tissues, with the highest repeatability coefficient observed for the parotid gland.

Summary: The study developed and externally validated NTCP models for various toxicities in head and neck cancer patients, involving 14 organs associated with at least one toxicity. The oral cavity was the predominant organ, while important organs included the parotid and submandibular glands, buccal mucosa, and swallowing muscles. The models showed a median discrimination performance of 0.71 at internal validation and 0.67 at external validation.

Dynamic Three-Dimensional ADC Changes of Parotid Glands During Radiotherapy Predict the Salivary Secretary Function in Patients With Head and Neck Squamous Carcinoma

Mei Feng et al.

Summary: The study investigated the changes in three-dimensional apparent diffusion coefficient (3D-ADC) of parotid glands in head and neck cancer patients during radiotherapy, and found correlations with radiation dose, volume reduction, and salivary secretary function. Results showed that the mean 3D-ADC increased and was negatively correlated with salivary function and volume reduction, but positively correlated with radiation dose. These dynamic 3D-ADC changes could serve as an early indicator for predicting and evaluating parotid gland function during radiotherapy.

Summary: This study focused on analyzing the molecular stability of MRI contrast agents when exposed to high energy photons and secondary electrons in a 1.5T MR-Linac system. The results showed no significant changes in chemical composition or displacement of gadolinium ions from their chelates in samples irradiated with 7 MV photons at clinical doses. Additionally, there was no significant correlation observed between concentrations of irradiated MRI contrast agents and radiation dose.

Radiomics feature stability of open-source software evaluated on apparent diffusion coefficient maps in head and neck cancer

James C. Korte et al.

Summary: This study investigates the reproducibility of radiomics features calculated with two widely used software packages (IBEX, MaZda) in comparison to an IBSI compliant software package (PyRadiomics). Only a subset of highly correlated features are considered reproducible, and including non-reproducible radiomics features in a HNC radiotherapy response model impacts the model's performance. Using radiomic features from different software packages can classify equivalent patient groups, but only when restricting the model to reliable features.
