Transplantation and Arch Repair in Fontan 3 Years After HeartMate 3: Technical Considerations

Summary: The article reports the successful transplantation of a Fontan patient after 1104 days of support with the HeartMate 3. It describes the operative technique used, which was complicated by the Fontan anatomy, ventricular assist device, and a large ascending and aortic arch aneurysm. The posttransplant hemodynamics of the patient showed that effective ventricular assist device support may reverse the chronic effects of the failing Fontan circulation and eliminate aortopulmonary collateral burden.

Summary: Despite tricuspid regurgitation (TR) and congenital heart disease (CHD) being exclusion criteria for the implantation of the Impella RP device, a clinical case of a 36-year-old patient with Ebstein's anomaly and severe TR supported with a percutaneous device as a bridge to heart transplant for 30 days is reported. The patient showed improvement both clinically and hemodynamically during this period, even though the device had to be removed due to displacement to the right ventricle. After mechanical unloading with partial right ventricle recovery, the patient's functional class improved.
