Hypothermia for Cardioprotection in Patients with St-Elevation Myocardial Infarction: Do Not Give It the Cold Shoulder Yet!

Mohamed El Farissi et al.

Summary: The timely revascularization of an occluded coronary artery is crucial in treating patients with STEMI, but it can also cause additional damage known as myocardial reperfusion injury. Unfortunately, there is still no effective treatment for this injury, despite numerous experimental therapies showing promise but failing to demonstrate the same results in human trials.

Door-to-unload: left ventricular unloading before reperfusion in ST-elevation myocardial infarction

Malav J. Parikh et al.

Summary: Treatment of ST-elevation myocardial infarction in the modern era focuses on reducing ischemic time through prompt reperfusion to limit infarct size and improve patient outcomes. Despite advancements in minimizing treatment time, heart failure incidence post-myocardial infarction remains high. Preclinical studies suggest that unloading the left ventricle prior to reperfusion could reduce infarct size and enhance myocardial recovery. Further research is ongoing to investigate this treatment strategy.

C-Reactive Protein Apheresis as Anti-inflammatory Therapy in Acute Myocardial Infarction: Results of the CAMI-1 Study

Wolfgang Ries et al.

Summary: This study aimed to investigate the relationship between CRP levels, myocardial infarct size and function, and the safety and efficacy of CRP apheresis in STEMI patients. The results showed significant differences in the correlation between CRP concentration and infarct size, as well as LV function, between the CRP apheresis group and the control group. CRP apheresis effectively reduced CRP levels without relevant side effects and has the potential to interfere with deleterious aspects of STEMI.

Summary: Despite the reduction in door-to-balloon times, the rates of death and heart failure in patients with ST-segment elevation myocardial infarction have plateaued, indicating a need to further reduce MI size. Localized delivery of supersaturated oxygen therapy has shown promise in reducing infarct size and improving cardiac function, making it a potential adjunctive therapy for myocardial salvage.
