Donor Clonal Hematopoiesis and Recipient Outcomes After Transplantation

Christopher J. Gibson et al.

Summary: Donor clonal hematopoiesis (CH) has a significant impact on clinical outcomes and graft alloimmune function in transplant recipients. DNMT3A-CH is associated with improved recipient survival and an augmented network of inflammatory cytokines in recipients. Different mutations may contribute to donor cell leukemia (DCL) in allogeneic hematopoietic cell transplantation.

Acute Myeloid Leukemia Case after Gene Therapy for Sickle Cell Disease

Sunita Goyal et al.

Summary: A woman developed acute myeloid leukemia 5.5 years after receiving LentiGlobin therapy for sickle cell disease, but investigation revealed that the leukemia was unlikely related to vector insertion. Other genetic mutations and inadequate disease control after treatment may contribute to the development of leukemia in sickle cell disease patients.

Clonal hematopoiesis in sickle cell disease

L. Alexander Liggett et al.

Summary: Through the study of whole genome sequences, we found no detectable increased risk of clonal hematopoiesis (CH) predisposing to myeloid malignancy in sickle cell disease (SCD). These results provide guidance for further research and characterization of risk factors underlying myeloid malignancy predisposition in SCD.

Absence of hydroxyurea-induced mutational effects supports higher utilisation for the treatment of sickle cell anaemia

Russell E. Ware et al.

Summary: Hydroxyurea is effective in treating SCA by inducing HbF and improving symptoms, but concerns about its mutagenic and carcinogenic risks have limited its broad use. However, current data suggest that hydroxyurea does not directly bind DNA or exhibit mutagenic activity.

Non-myeloablative human leukocyte antigen-matched related donor transplantation in sickle cell disease: outcomes from three independent centres

Mohsen Alzahrani et al.

Summary: Non-myeloablative HPCT from MRD has shown to be effective and well-tolerated in treating SCD patients, with high survival rates and low incidence of clinically significant GVHD. Organ functions improved post-transplantation and further sickle-related injuries were halted, suggesting the success of the treatment regimen.

Increased stem cell proliferation in atherosclerosis accelerates clonal hematopoiesis

Alexander Heyde et al.

Summary: Clonal hematopoiesis, a condition where certain hematopoietic stem cell clones generate a disproportionate fraction of blood leukocytes, is correlated with increased risk for cardiovascular disease. Research shows that in individuals with atherosclerosis, hematopoietic stem cell division rates are elevated, leading to an increased risk of clonal hematopoiesis. The study suggests that increased hematopoietic stem cell proliferation is an important factor contributing to the association between cardiovascular disease and clonal hematopoiesis.

Ineffective erythropoiesis in sickle cell disease: new insights and future implications

Summary: SCD is a hemolytic anemia caused by abnormal hemoglobin leading to red cell sickling. Recent research suggests that defects in erythropoiesis may also contribute to anemia in SCD patients, highlighting potential implications for therapeutic strategies.

Chronic infection drives Dnmt3a-loss-of-function clonal hematopoiesis via IFNγ signaling

Daniel Hormaechea-Agulla et al.

Summary: The study demonstrates that IFN gamma signaling induced during chronic infection can drive DNMT3A-loss-of-function clonal hematopoiesis. This suggests that environmental factors play an important role in the development of CH.

Leukemia after gene therapy for sickle cell disease: insertional mutagenesis, busulfan, both, or neither

Richard J. Jones et al.

Summary: Recent data provides hope for gene therapy as a cure for SCD, but concerns were raised after the suspension of a gene therapy trial due to AML/MDS development. A new hypothesis suggests that gene therapy may drive leukemogenic transformation through clonal expansion of premalignant clones. Alternative therapies are being explored in multicenter trials, with promising preliminary results.
