Far-red light-activated human islet-like designer cells enable sustained fine-tuned secretion of insulin for glucose control

Guiling Yu et al.

Summary: Diabetes affects a large number of people worldwide, and most individuals with T1D and T2D rely on self-administration of insulin for glucose control. This study demonstrates the development of far-red light activated cells that can achieve safe and sustained glucose control when implanted in diabetic model mice. The cells were triggered by optogenetics and showed improvement in glucose tolerance and reduction in diabetes-related complications.

Effects of glucose variability on hematopoietic stem/progenitor cells in patients with type 1 diabetes

Summary: This study demonstrates the depletion of HSPCs in T1D patients and highlights a potential stimulatory effect of hypoglycaemia on HSPCs, which diminishes with longer duration of diabetes. This suggests that chronicity of the disease may progressively reduce the release of HSPCs in response to adrenergic triggers.

Summary: The study found that hUCMS inhibited proliferation of allogeneic polymorphonuclear blood cells from patients with T1D, while promoting expansion of FoxP3(+)Tregs. Through co-microencapsulation, hUCMS and hIDC synergized anatomically and functionally, maintaining tracer insulin output by hIDC and utilizing the immunoregulatory properties of hUCMS to help decrease blood glucose levels.

Intrapancreatic MSC transplantation facilitates pancreatic islet regeneration

Rahul Khatri et al.

Summary: The study suggests that the physical contact between MSC and pancreatic islet cells is necessary for unleashing their protective potential fully. Administration of MSC through the intrapancreatic route improved hyperglycemia, promoted proliferation of islet cells, increased islet number and area, enhanced expression of growth factor (EGF), and balanced the Th1/Th2 response in vivo. Additionally, direct physical contact provided superior protection to MIN6-cells from STZ damage in vitro through the AKT and ERK pathways.

One repeated transplantation of allogeneic umbilical cord mesenchymal stromal cells in type 1 diabetes: an open parallel controlled clinical study

Jing Lu et al.

Summary: The study demonstrated that one repeated intravenous dose of allogeneic UC-MSCs is safe in people with recent-onset T1D and may result in better islet beta cell preservation during the first year after diagnosis compared to standard treatment alone. The MSC-treated group showed a significantly higher rate of achieving clinical remission compared to the control arm, with some individuals even achieving insulin independence. Multivariable logistic regression analysis showed that lower fasting C-peptide and higher dose of UC-MSC correlated with the achievement of clinical remission after transplantation, with no severe side effects observed.

Summary: Insulin injection and human pancreatic islet transplantation are current main therapies for T1D or late-stage severe T2D, but limitations such as islet shortage and immunosuppression side effects call for the need to develop novel methods for generating protected functional human islets. Human pluripotent stem cells offer a potentially limitless cell source, and studying immune tolerance induction in hPSC-derived human pancreatic islets contributes to the goal of curing insulin-dependent diabetes.
