A phase 3 randomized clinical trial using a once-weekly glucagon-like peptide-1 receptor agonist in adolescents and young adults with hypothalamic obesity

Christian L. Roth et al.

Summary: The study aimed to evaluate the efficacy and safety of GLP-1 RA in patients with HO. Although ExQW did not significantly change BMI after 36 weeks, it showed significant reductions in total body fat mass and waist circumference compared to placebo. GLP-1 RAs are a promising and safe treatment for improving or stabilizing HO in children and young adults.

Multidisciplinary Approach for Hypothalamic Obesity in Children and Adolescents: A Preliminary Study

Daniele Tessaris et al.

Summary: A multidisciplinary approach was used to treat overweight or obese children and adolescents with hypothalamic disease, leading to a significant reduction in weight gain and stabilization of weight and BMI standard deviation scores over a 6-month period. Patients treated with metformin also showed improved weight outcomes. Further analysis and longer follow-up are needed to confirm these initial results.

Oxytocin in young children with Prader-Willi syndrome: Results of a randomized, double-blind, placebo-controlled, crossover trial investigating 3 months of oxytocin

Layla Damen et al.

Summary: The study on the effects of intranasal oxytocin treatment on children with Prader-Willi syndrome showed positive effects on social and eating behavior, especially in boys aged 3-11 years and in children with a deletion, with no safety concerns. However, individual effects should be carefully evaluated.

The critical role of CCK in the regulation of food intake and diet-induced obesity

Carolina R. Cawthon et al.

Summary: CCK plays a crucial role in regulating feeding behavior and optimizing digestion and nutrient absorption. Diet-induced obesity results in reduced sensitivity to CCK, potentially leading to dysfunction of the CCK system.

Oxytocin and Food Intake Control: Neural, Behavioral, and Signaling Mechanisms

Summary: Oxytocin, produced in the hypothalamus, reduces food intake and holds potential for obesity treatment. Research in rodent models suggests that oxytocin reduces food intake by enhancing physiological satiation signals within meals.

Summary: The study found that greater hypothalamic damage, especially involving the mammillary body (MB), is associated with greater reductions in adiposity in patients with hypothalamic obesity who received treatment with GLP-1RA.

Setmelanotide: First Approval

Targeting Energy Expenditure-Drugs for Obesity Treatment

Carlos M. Jimenez-Munoz et al.

Summary: Obesity and overweight are linked to deadly diseases, with obese and overweight individuals infected by COVID-19 being at higher risk of mortality. Pharmaceutical approaches to treating obesity, such as appetite suppression, reducing energy intake, and increasing energy expenditure, have led to the development of compounds specifically targeting obesity by increasing energy expenditure. This review categorizes these compounds based on their development status and provides details on their chemical nature, target, mechanisms of action, and development stage.

Sleep disturbances in craniopharyngioma: a challenging diagnosis

Ramona Cordani et al.

Summary: Craniopharyngiomas can lead to significant neurological and endocrinological complications, including disturbances in sleep-wake regulation, altered sleep structure, and changes in sleep-wake rhythm. Accurate diagnosis and differential diagnosis of sleep disorders are crucial in patients with this type of tumor.

Brain insulin signalling in metabolic homeostasis and disease

Thomas Scherer et al.

Summary: Insulin signalling in the central nervous system regulates energy homeostasis by controlling metabolism in several organs and coordinating organ crosstalk. Studies in rodents, non-human primates, and humans show that brain insulin action can reduce food intake and regulate energy balance by orchestrating nutrient partitioning, while brain insulin resistance contributes to metabolic diseases like obesity, metabolic syndrome, and type 2 diabetes mellitus.

Risk Factors for Hypothalamic Obesity in Patients With Adult-Onset Craniopharyngioma: A Consecutive Series of 120 Cases

Summary: The study found that adult-onset patients with craniopharyngioma are at risk for significant weight gain postoperatively, especially those with lower preoperative BMI and the adamantinomatous subtype. The prevalence of HO after craniopharyngioma surgery is relatively high, with only preoperative BMI identified as an independent risk factor for postoperative HO.

The efficacy and safety of setmelanotide in individuals with Bardet-Biedl syndrome or Alstrom syndrome: Phase 3 trial design

Robert M. Haws et al.

Summary: This is a phase 3 trial investigating the efficacy and safety of the MC4R agonist setmelanotide for treating obesity and hyperphagia in individuals with BBS and Alstrom syndrome. The study aims to enroll 30 participants with the primary endpoint being the proportion of participants achieving a clinically meaningful reduction in body weight after approximately 52 weeks. Safety and tolerability will be assessed by frequency of adverse events.
