Empirical Treatment With Carbapenem vs Third-generation Cephalosporin for Treatment of Spontaneous Bacterial Peritonitis

Sun Woong Kim et al.

Summary: Empirical treatment with carbapenem does not reduce in-hospital mortality compared to treatment with TGCs for patients with a first presentation of SBP. However, among critically ill patients with high CLIF-SOFA scores, empirical carbapenem treatment was significantly associated with lower in-hospital mortality than TGCs.

Summary: The study examined Gram-negative bacterial bloodstream infections at a tertiary-care hospital in Beijing, China. The overall mortality of BSI cases decreased slightly, with Escherichia coli being the major pathogen and showing higher resistance to cephalosporins and carbapenems.

Summary: This study compared the efficacy of non-carbapenem beta-lactam/beta-lactamase inhibitors (BLBLIs) with carbapenems for the treatment of UTIs caused by ESBL-PE. The results showed no significant difference in clinical success and mortality between the two treatment groups, but a slightly higher rate of microbiological success was observed in the BLBLIs group.

Healthcare-associated carbapenem-resistant Klebsiella pneumoniae bloodstream infections: Risk factors, mortality, and antimicrobial susceptibility, 2017-2019

Summary: This study found that prior exposure to carbapenems, longer hospital stay, and the presence of liver cirrhosis predicted CRKP infection. CRKP isolates were less susceptible to antibiotics compared to CSKP isolates, and CRKP infection was associated with significantly lower 14-day survival rates. Ceftazidime/avibactam may be a useful therapeutic option for infections caused by CRKP strains that are susceptible in vitro.

Draft Genome Sequence and Biofilm Production of a Carbapenemase-Producing Klebsiella pneumoniae (KpR405) Sequence Type 405 Strain Isolated in Italy

Teresa Fasciana et al.

Summary: The rapid identification and characterization of multidrug-resistant Klebsiella pneumoniae strains is crucial for diagnosing severe infections in patients. Traditional methods such as pulsed-field gel electrophoresis or multilocus sequence typing are time-consuming, laborious, and expensive, while next-generation sequencing has emerged as a faster and more cost-effective alternative.
