Risk factors for acquisition of colistin-resistant Klebsiella pneumoniae and expansion of a colistin-resistant ST307 epidemic clone in hospitals in Marseille, France, 2014 to 2017

Sophie Alexandra Baron et al.

Summary: This study in Marseille, France identified male sex and contact with a patient carrying CRKP as significant risk factors for CRKP acquisition, while colistin administration was not found to be a key factor. A specific ST307 CRKP clone was identified with chromosomal mutations potentially linked to colistin resistance, suggesting a propensity for resistance that warrants further investigation.

In vivo Emergence of Colistin Resistance in Carbapenem-Resistant Klebsiella pneumoniae Mediated by Premature Termination of the mgrB Gene Regulator

Yingying Kong et al.

Summary: This study isolated a pair of carbapenem-resistant Klebsiella pneumoniae isolates from the same patient before and after colistin treatment, named KP1-1 and KP1-2. The Gln30* alteration in KP1-2 was found to be responsible for colistin resistance, and complementation with wild-type mgrB gene restored colistin susceptibility. Overexpression of phoP, phoQ, and pmrD genes in KP1-2 compared to KP1-1 confirmed the role of MgrB alterations in increased expression levels of these genes, providing direct in vivo evidence for colistin resistance in K. pneumoniae clinical strains.
