This review of 17 clinical trials of anti-CD19 CAR-T cell therapies highlights the association of the CD28 structural hinge and transmembrane CAR domains with both clinical efficacy and severe toxicity. Further investigation into structural CAR domains is recommended for improved CAR design and safer therapies.
