Epidemiology of the inherited cardiomyopathies

William J. McKenna et al.

Summary: Estimates of the incidence and prevalence of inherited cardiomyopathies are challenging due to incomplete and late-onset disease expression, highly variable penetrance within families, and overlapping phenotypes. Current global estimates for hypertrophic, dilated, and arrhythmogenic cardiomyopathies may be conservative.

Hypertrophic Cardiomyopathy: Genetic Testing and Risk Stratification

Fergus Stafford et al.

Summary: Our understanding of the genetic basis and molecular pathogenesis of hypertrophic cardiomyopathy (HCM) is continuously evolving, with recent advances in genetic testing showing the presence of important HCM sub-groups. Genetic testing has emerged as a valuable tool for cascade genetic testing and may play a significant role in prognosis and patient management in the future. Genotype may potentially enhance risk stratification, management, treatment, and prognosis in HCM, leading to improved outcomes for patients and their families.

A cryptic splice-altering KCNQ1 variant in trans with R259L leading to Jervell and Lange-Nielsen syndrome

Mario Torrado et al.

Summary: An infant with JLNS was reported with two KCNQ1 variants identified, one known pathogenic missense variant and a novel intronic variant. Family analysis and minigene splicing assay confirmed the intronic variant induced mis-splicing, leading to a truncated protein. This study expanded the genetic spectrum of biallelic KCNQ1 variants causing JLNS1.

Benchmarking deep learning splice prediction tools using functional splice assays

Tabea V. Riepe et al.

Summary: Hereditary disorders are often caused by genetic variants affecting RNA splicing, but predicting the pathogenicity of variants in noncanonical splice sites and deep intronic regions is challenging. A study comparing splice prediction tools on variants in two genes found that SpliceRover and SpliceAI performed the best.

Protein haploinsufficiency drivers identify MYBPC3 variants that cause hypertrophic cardiomyopathy

Carmen Suay-Corredera et al.

Summary: This study examined 44 nontruncating MYBPC3 variants and found that around half of the HCM-linked variants exhibited alterations in RNA splicing or protein stability, leading to cMyBP-C haploinsufficiency. Additionally, some nontruncating MYBPC3 variants classified as of uncertain significance also induced one or both molecular phenotypes.

Role of Splicing Regulatory Elements and In Silico Tools Usage in the Identification of Deep Intronic Splicing Variants in Hereditary Breast/Ovarian Cancer Genes

Alejandro Moles-Fernandez et al.

Summary: This study characterized intronic regions and tested the efficiency of computational tools to detect deep intronic variants affecting mRNA splicing. Results showed limited effectiveness in predicting variants affecting splicing regulatory elements. The combination of SpliceAI and ESRseq scores showed higher sensitivity but more false positives, indicating room for improvement in detecting deep intronic variants disrupting splicing.

Pathogenic Intronic Splice-Affecting Variants in MYBPC3 in Three Patients with Hypertrophic Cardiomyopathy

Katherine A. Wood et al.

Summary: This study identified three patients with HCM and intronic splice-affecting MYBPC3 variants, analyzing the impact of variants on splicing and suggesting the reclassification of two variants as likely pathogenic. The findings highlight the need for functional assessment of variants of uncertain significance in diagnostic testing and the limitations of commonly applied bioinformatics strategies in prioritizing variants impacting MYBPC3 splicing.
