The vascular occlusion test using multispectral imaging: a validation study The VASOIMAGE study

Arnoud A. Bruins et al.

Summary: The study validated the potential of multispectral imaging in detecting microcirculatory perfusion disturbances during a vascular occlusion test, showing that it can measure changes in microcirculatory perfusion in healthy volunteers. However, the correlation of results between multispectral imaging and near-infrared spectroscopy was moderate to weak, possibly due to differences in physiology of the regions of interest and measurement techniques.

Increase in regional cerebral saturation after elective electrical cardioversion of atrial fibrillation is only transient and without beneficial effects on neuropsychological functioning: cerebral saturation during electrical cardioversion

Cornelia Genbrugge et al.

Summary: The study confirmed a positive association between successful electrical cardioversion and an increase in SctO(2), but the increase was temporary and did not affect the neuropsychological function of patients. Successful electrical cardioversion improved the quality of life of patients, but did not have an impact on neuropsychological functioning at follow-up.

Evaluation of accuracy of capillary refill index with pneumatic fingertip compression

Koichiro Shinozaki et al.

Summary: Capillary refill time (CRT) is commonly used to evaluate peripheral perfusion status, but a new method called capillary refill index (CRI) using pulse oximetry sensor was developed in this study. CRI showed strong correlation and reliability similar to CRT by image analysis, but with approximately 1 second higher values.

Ocular blood flow by laser speckle flowgraphy to detect cerebral ischemia during carotid endarterectomy

Yasushi Motoyama et al.

Summary: LSFG can measure ocular blood flow velocity to detect ischemic stress during CEA. The MBR reduction ratio is positively correlated with the rSO(2) index, suggesting LSFG could be used to evaluate cerebral ischemic tolerance during CEA.

Regional cerebral oximetry is consistent across self-reported racial groups and predicts 30-day mortality in cardiac surgery: a retrospective analysis

Blaine Stannard et al.

Summary: Different racial groups may show differences in regional oxygen saturation measurements during cardiac surgery, but these differences are less than 2% after adjusting for perioperative variables. Cerebral oximetry measurements are consistent across races, supporting their use for intraoperative monitoring and risk stratification. Pre-intervention oxygen saturation levels are associated with increased 30-day mortality, primarily driven by the numerical values rather than racial factors.

A new photoplethysmographic device for continuous assessment of urethral mucosa perfusion: evaluation in a porcine model

Martina Cardinali et al.

Summary: This study evaluated an innovative device using a photoplethysmography (PPG) sensor in contact with the urethral mucosa to continuously monitor the urethral perfusion index (uPI) in a porcine model. The results showed a positive correlation between uPI and mean arterial pressure (MAP) below a specific threshold, and a negative correlation above that threshold. The device was able to detect urethral mucosa perfusion alterations associated with hypotension or excessive doses of vasopressors, suggesting potential clinical applications.

Summary: The study compared the image acquisition quality between the new MS-U and old MS-A devices. MS-U showed superior image quality in terms of illumination, focus, and pressure, with good agreement between raters.

Comparing the effect of positioning on cerebral autoregulation during radical prostatectomy: a prospective observational study

Stefanie Beck et al.

Summary: Robot-assisted radical prostatectomy in the extreme Trendelenburg position did not impair cerebral autoregulation compared with open retropubic radical prostatectomy in the supine position. Age and intraoperative mean arterial blood pressure difference were associated with impaired cerebral autoregulation, while surgical technique was not.

Laser speckle contrast imaging of forehead cutaneous blood flow during carotid endarterectomy as a potential non-invasive method for surrogate monitoring of cerebral perfusion

Mads J. Niemann et al.

Summary: Monitoring changes in cutaneous blood flow of the supraorbital region and Glabella using LSCI during carotid endarterectomy under local anesthesia reflected a decrease in blood flow during clamping of the common carotid artery, but did not differentiate between reperfusion of the internal carotid artery and external carotid artery.

Effects of anesthetics on microvascular reactivity measured by vascular occlusion tests during off-pump coronary artery bypass surgery: a randomized controlled trial

Youn Joung Cho et al.

Summary: During off-pump coronary artery bypass surgery, there were no significant differences in microvascular reactivity between patients receiving sevoflurane and desflurane. Additionally, both groups showed similar macrohemodynamics and rates of postoperative adverse events, suggesting comparable effects of the two anesthetics on microvascular function in this setting.

Summary: The study investigated the relationship between lactate clearance and PtcO2-related variables in septic patients, finding significant correlations between lactate clearance and P(tc)O(2) at T6, Delta PtcO2, Delta P(tc)O(2)index, and Delta 10 OCT. Delta PtcO2 showed the best performance in discriminating low lactate clearance groups.

Perfusion index: Physical principles, physiological meanings and clinical implications in anaesthesia and critical care

Maxime Coutrot et al.

Summary: Perfusion index (PI), derived from photoplethysmography (PPG) signal, represents the ratio of pulsatile to non-pulsatile light absorbance or reflectance. The determinants of PI are complex and interlinked, involving the interaction between peripheral and central haemodynamic characteristics. Studies have shown PI's potential in assessing regional or neuraxial block success and haemodynamic monitoring in various clinical settings.

Summary: This guideline updates and replaces the 5th edition of the Standards of Monitoring published in 2015, providing guidance on minimum monitoring standards for patients undergoing anaesthesia or sedation. It includes standards for monitoring patients during anaesthesia and in the recovery phase, as well as guidance on monitoring patients undergoing sedation and during transfer. New sections on capnography, sedation, and regional anaesthesia have been added, along with updated indications for processed electroencephalogram and neuromuscular monitoring.
