The Role of Bone Volume, FGF23 and Sclerostin in Calcifications and Mortality; a Cohort Study in CKD Stage 5 Patients

Summary: This study investigated the prevalence and phenotype of bone disease in kidney transplant patients, and examined the associations between FGF23, sclerostin, bone histomorphometry, cardiovascular disease, and mortality. The results showed that age, sclerostin levels, and other factors were associated with different types of bone diseases. High levels of FGF23 and sclerostin were found to be correlated with higher cardiovascular risk and involvement of low bone volume in extra-osseous calcifications.

Summary: CKD-MBD after kidney transplantation is characterized by abnormal mineral metabolism and bone alterations, with hypercalcemia primarily caused by high levels of PTH and hypophosphatemia caused by high levels of PTH and/or FGF23. Treatment options include calcimimetics. The most common bone morphology alteration is low bone turnover disease, and osteoporosis is a major risk factor for fractures post-transplantation.
