Effect of Intra-arterial Alteplase vs Placebo Following Successful Thrombectomy on Functional Outcomes in Patients With Large Vessel Occlusion Acute Ischemic Stroke The CHOICE Randomized Clinical Trial

Arturo Renu et al.

Summary: This study investigated whether treatment with adjunct intra-arterial alteplase after thrombectomy improves outcomes following reperfusion. The results showed that compared with placebo, the use of adjunct intra-arterial alteplase resulted in a greater likelihood of excellent neurological outcome at 90 days.

European Stroke Organisation (ESO)-European Society for Minimally Invasive Neurological Therapy (ESMINT) expedited recommendation on indication for intravenous thrombolysis before mechanical thrombectomy in patients with acute ischemic stroke and anterior circulation large vessel occlusion

Guillaume Turc et al.

Summary: This study evaluated whether mechanical thrombectomy (MT) alone is non-inferior to intravenous thrombolysis (IVT) plus MT in patients with anterior circulation large vessel occlusion (LVO) ischemic stroke. The recommendations suggest using IVT plus MT for patients directly admitted to a MT-capable center within 4.5 hours of symptoms onset, and IVT followed by rapid transfer to a MT-capable center for patients admitted to a center without MT facilities.

Mechanical Thrombectomy vs Bridging Therapy for Anterior Circulation Large Vessel Occlusion Stroke Systematic Review and Meta-analysis

Gabriela Trifan et al.

Summary: The study compared the efficacy and safety of bridging therapy (BT) and mechanical thrombectomy (MT) for anterior circulation large vessel occlusion stroke, finding that the BT group had better outcomes in terms of functional independence, successful recanalization rates, and mortality, with similar rates of symptomatic intracranial hemorrhage. Further research is still needed.

Summary: In this study, there was no evidence found that occlusion location could influence decisions on intravenous alteplase in endovascular treatment-eligible patients presenting directly at endovascular treatment-capable centers. Further research is needed to validate these findings.

Endovascular therapy with or without intravenous thrombolysis in acute stroke with tandem occlusion

Mohammad Anadani et al.

Summary: This study compared the treatment effects of EVT alone and EVT plus IVT in patients with acute ischemic stroke due to anterior tandem occlusions. The results showed higher odds of favorable and excellent outcomes, as well as successful reperfusion in the IVT+ group, without a difference in the risk of significant hemorrhagic complications. Secondary analysis also indicated better outcomes with bridging therapy in patients treated with acute cervical internal carotid artery stenting.

Does prior administration of rtPA influence acute ischemic stroke clot composition? Findings from the analysis of clots retrieved with mechanical thrombectomy from the RESTORE registry

Rosanna Rossi et al.

Summary: This study aimed to investigate the effect of bridging therapy (IVT + MT) versus MT alone on the size and histological composition of clots retrieved from patients with acute ischemic stroke. The results showed that patients receiving bridging therapy had significantly smaller clots, as well as less red blood cells, fibrin, and platelets compared to those undergoing MT alone, indicating that IVT can significantly reduce thrombus size.

Summary: This study aimed to investigate the association between intravenous thrombolysis (IVT) before endovascular thrombectomy (EVT) and the safety and efficacy of EVT for large vessel occlusion stroke. The results showed that preceding IVT was associated with better functional outcomes. This finding provides evidence for further evaluation.

Summary: The study aimed to develop and validate an easily applicable predictive score for early neurological deterioration following intravenous thrombolysis in patients with acute minor stroke and large vessel occlusion. The results showed that a score based on thrombus length and occlusion site had good discriminative power for identifying ENDi risk, with higher scores associated with greater probability of ENDi occurring.

Summary: The extent of early ischemia may influence relative risks of sICH in IVT+MT versus direct MT patients, with an excess sICH risk in IVT+MT patients with low ASPECTS. If confirmed in post hoc analyses of randomized controlled trial data, IVT may be administered more carefully in patients with low ASPECTS eligible for and with direct access to MT.
