Summary: Following prenatal diagnosis of congenital diaphragmatic hernia, the severity can be predicted based on associated abnormalities, lung size, and position of the liver. Fetoscopic endoluminal tracheal occlusion (FETO) can reverse pulmonary hypoplasia and improve neonatal outcome. Recent randomized controlled trials showed that FETO increased survival to discharge from the neonatal intensive care unit in severe and moderate cases, but with a higher risk of prematurity.

Summary: This article aims to describe the methodology for defining a COS for perinatal and neonatal outcomes of fetuses and newborns with congenital diaphragmatic hernia and to draft a dissemination and implementation plan. The methodology described in the COMET Initiative Handbook will be used. An international steering group will be established to guide the development of the COS. The literature is being systematically reviewed to identify all potential relevant pre- and neonatal outcomes previously used in studies on perinatal interventions for CDH.

Future research should further explore these concepts for potential advancements in care.

Summary: Fetal lung underdevelopment, also known as pulmonary hypoplasia, is characterized by decreased lung growth and maturation, with congenital diaphragmatic hernia being the most common birth defect associated with it. Despite the lack of effective treatment options, a stem cell-based approach involving the administration of extracellular vesicles derived from amniotic fluid stem cells has shown promise in enhancing fetal lung development in rodent models. These vesicles can promote branching morphogenesis and alveolarization, potentially offering a regenerative solution for underdeveloped fetal lungs.
