From less invasive to minimal invasive extracorporeal circulation

Summary: The development of minimally invasive cardiac surgery aims to reduce surgical stress triggered by full median sternotomy, minimize surgical trauma, improve recovery, and lower morbidity. Extracorporeal circulation is fundamental to most MICS procedures, with advancements in perfusion techniques enabling the development of MICS. Less invasive cardiopulmonary bypass techniques focus on facilitating demanding surgical maneuvers rather than minimizing invasiveness, leading to increased CPB duration for MICS procedures. Minimal invasive Extracorporeal Circulation (MiECC) represents a major breakthrough in perfusion, translating to improved clinical outcomes and end-organ protection. Integration of MiECC can advance MICS towards a more physiologic approach to cardiac surgery.

Comparative Effects of Single-Dose Cardioplegic Solutions Especially in Repeated Doses During Minimally Invasive Aortic Valve Surgery

Summary: The study compared del Nido cardioplegia (DNC) and histidine-tryptophan-ketoglutarate (HTK) cardioplegic solutions in minimally invasive aortic valve replacement surgery, finding that both provided acceptable myocardial protection as single-dose applications. However, patients who needed redosing showed significantly unbalanced biomarker levels and more arrhythmia/AV block incidence in the DNC-R group, resulting in longer ICU stay. HTK may be preferable to DNC for patients who may need redosing.
