Summary: Posaconazole is a promising antifungal agent with favorable safety and tolerability profile. It has extended spectrum of activity against yeasts and filamentous fungi. It is successfully used for prophylaxis in patients with acute myeloid leukemia/myelodysplastic syndrome and post-hematopoietic cell transplantation with graft-versus-host disease, as well as first line treatment for oropharyngeal candidiasis and salvage treatment for refractory invasive fungal diseases.

Summary: Posaconazole has higher activity against Candida albicans than fluconazole in vitro. The efficacy of posaconazole against C. albicans in an in vitro pharmacokinetic/pharmacodynamic (PK/PD) model of invasive candidiasis was explored. Different probabilities of pharmacodynamic target attainment were observed for oral solution and intravenous/tablet formulations. Non-wild-type isolates with higher MICs may require higher trough levels for optimal treatment.

Recent research has shown that metabolism plays a crucial role in controlling parasite tropism and the severity of infection in hosts. Further studies are needed to explore in vivo parasite metabolic gene expression, the relationship between local metabolic perturbation, parasite strain, disease location, and host-parasite-microbiota co-metabolism.

Summary: The study evaluated the activity of posaconazole against invasive moulds worldwide, showing similar effectiveness against common Aspergillus species but high resistance in Fusarium and Scedosporium. A gene called LmjPES, similar to the human oncogene PES1, plays a role in cell processes and showed increased infection rates when overexpressed in parasites. Mice infected with parasites overexpressing LmjPES had higher inflammation levels.

Caution should be exercised when using these drugs, with an understanding of their pharmacokinetics, safety, and drug-drug interactions to ensure effective antifungal therapy. The manuscript reviews significant drug interactions of azoles with other medications, as well as with food, and highlights possible interactions during experimental therapies for the treatment of COVID-19.
